Help make behavioral health care accessible when clients need it most. What happens when someone is discharged from the hospital after a serious medical event, recovering following surgery, experiencing significant or acute medical challenges or other serious medical events, but their mental health needs don't stop and getting to a traditional therapy appointment simply isn't realistic? That's where Integrated Care comes in. Center for Life Management is seeking an Integrated Care Therapist to provide individual and group therapy to individuals living with serious and persistent mental health conditions and co-occurring chronic physical health conditions that significantly affect their daily functioning and ability to access care. This is a clinical role for someone who believes effective behavioral health treatment means meeting clients where they are. That may mean seeing a client in our office. It may mean providing therapy through telehealth. And when significant medical or functional challenges prevent someone from accessing care in those settings, it may mean meeting them in their home or another community setting. We help clients build greater independence, reconnect with their communities, and access treatment in the least restrictive setting appropriate for them. At the same time, we recognize that clients living with complex or chronic medical conditions sometimes need us to bring care to them.

As an Integrated Care Therapist, you'll provide meaningful, person-centered clinical care while working with clients whose behavioral and physical health needs intersect. You'll:

• Provide individual and group therapy to adults with serious and persistent mental health conditions and co-occurring chronic physical health conditions.
• Conduct behavioral health assessments and develop individualized treatment plans.
• Help clients understand and manage symptoms, strengthen coping skills, improve functioning, and work toward greater independence.
• Provide services flexibly in the office, clients' homes and community settings, and through telehealth based on clinical needs and treatment goals.
• Support clients whose acute or chronic medical circumstances may create barriers to participating in traditional office-based behavioral health treatment.
• Provide crisis assessment and intervention when clients experience acute behavioral health needs.
• Collaborate with medical and behavioral health providers, families, natural supports, and other members of the treatment team to coordinate care.
• Provide psychoeducation and evidence-based interventions that help clients manage the relationship between their behavioral health, physical health, and daily functioning.
• Help clients remain connected to treatment during difficult periods and, whenever possible, move toward greater independence and participation in their communities.

Integrated Care uses a flexible hybrid and community-based model. A typical week may include 2 days in the CLM office, 2 days providing services in the community or clients' homes, and 1 day providing telehealth services remotely. But Integrated Care isn't a one-size-fits-all program, and neither is the schedule. Your week may change based on the needs of the people you're serving. You might begin your day providing telehealth sessions and later meet a client in the community because their circumstances make another form of treatment inaccessible. That flexibility is an important part of the position and an important part of how we remove barriers to behavioral health care.

We're looking for a therapist who wants to do more than provide therapy within the four walls of an office. You may be a great fit if you:

• Hold a master's degree in social work, clinical mental health counseling, psychology, or a related mental health field.
• Are an LCMHC, LICSW, or license-eligible master's-level therapist (preferred but not required).
• Want to work with individuals experiencing serious mental health conditions alongside chronic or complex physical health needs.
• Believe people should be able to access quality behavioral health treatment even when health, transportation, mobility, or other circumstances make traditional services difficult to access.
• Enjoy working collaboratively with other professionals.
• Are comfortable providing services in a variety of settings, including the office, community, clients' homes, and through telehealth.
• Can respond thoughtfully and clinically to both ongoing treatment needs and more acute situations.
• Are curious about the connection between mental health, physical health, and overall functioning.

You do not need to come to us as an expert in complex medical conditions or Integrated Care. Experience with medically complex populations is valuable, but we're equally interested in therapists who want to learn and expand their practice. Physical Demands and Working Conditions

The work will be performed in the community, client’s home, office or remotely via telehealth depending on the treatment objectives and interventions. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable internet access, transportation, a clean driving record, and proof of auto insurance coverage is required.
