Golden Years ADHC is seeking a full-time Care Coordinator to join our Edmonton team. You will help participants and caregivers stay connected, respond when services do not go as planned, monitor authorized and scheduled service hours, complete participant follow-up, and make sure important concerns reach the right person.
This is not a position where every day looks exactly the same. Participant needs, caregiver call-offs, home visits, incidents, and service gaps can quickly change the day. We are looking for someone who can calmly adjust priorities while continuing to communicate, document, and follow through.
Required Qualifications
High school diploma or GED.
Strong communication, organization, time-management, and follow-through skills.
Ability to communicate respectfully with participants, families, caregivers, case managers, and team members.
Comfort using email, Microsoft Office, electronic documentation systems, and standard office technology.
Ability to document facts clearly, accurately, and on time.
Valid driver’s license, reliable transportation, and current automobile insurance.
Ability to travel locally for participant visits and service follow-up.
Ability to work full-time from our Edmonton office and work one Saturday per month.
Ability to complete all screenings, training, and credentialing required for the position.
Preferred Experience
At least one year of experience in healthcare, home care, Medicaid waiver services, care coordination, scheduling, human services, social services, participant support, or a related field.
Experience supporting older adults, adults with disabilities, caregivers, or families.
Familiarity with Kentucky Medicaid, Home and Community-Based waiver services, attendant care, service authorizations, or person-centered support.
Experience addressing caregiver call-offs, missed services, scheduling problems, participant concerns, or rapidly changing priorities.
What You Will Do
Build respectful, dependable relationships with participants, families, responsible representatives, caregivers, and community partners.
Coordinate assigned participant services according to approved authorizations, service plans, schedules, and Golden Years procedures.
Monitor authorized, scheduled, delivered, missed, and recovered service hours.
Respond promptly to caregiver call-offs, participant refusals, hospitalizations, rehabilitation stays, vacations, and other service interruptions.
Work with scheduling and leadership to address approved open hours and prevent avoidable gaps in care.
Complete assigned participant phone follow-ups, home visits, assessments, and service-quality checks.
Communicate service gaps, changes in condition, participant concerns, and unresolved needs to the appropriate supervisor and conflict-free case manager.
Document contacts, attempted contacts, notifications, actions taken, outcomes, and next steps in approved systems.
Respond immediately to urgent health or safety concerns and follow required incident and reporting procedures.
Help caregivers understand schedules, participant-specific expectations, documentation requirements, and call-off procedures.
Support participant choice, dignity, privacy, and independence without pressure, steering, or retaliation.
Assist with service starts, participant transitions, caregiver introductions, transfers, and discharges according to approved procedures.
Your First 90 Days
You will receive structured onboarding and hands-on training in Golden Years systems, participant communication, service-hour monitoring, home and phone follow-up, incident reporting, documentation, privacy, person-centered practices, and Kentucky waiver expectations.
You will work with experienced leaders and team members while demonstrating each required competency. You will not be expected to learn the position alone, and your training will focus on helping you become confident, dependable, and prepared to support participants and caregivers.

Participants and caregivers receive timely, respectful communication.
Service gaps are identified early and followed through.
Authorized services are scheduled and delivered as consistently as possible.
Documentation is complete, factual, timely, and ready for review.
Urgent concerns are escalated immediately through the correct channels.
Caregivers understand expectations and receive clear support.
You remain calm, present, and solution-focused when priorities change.
Pay, Schedule, and Growth
The hourly pay range is $20.00 to $24.00, based on directly related experience, qualifications, and demonstrated competency. Candidates with hands-on home-care, Medicaid waiver, scheduling, or care-coordination experience may qualify for the higher portion of the range.
This is a full-time position based in our Edmonton office. The normal schedule is primarily Monday through Friday, and one Saturday per month is required. Occasional schedule adjustments may be necessary when participant needs, incidents, caregiver call-offs, or service gaps require timely support.
The position offers exposure to Golden Years’ Adult Day Healthcare, Attendant Care, NEMT Transportation, Conflict-Free Case Management, VA, and Private Pay service lines.
