OVERVIEW

A Contractor Person-Centered Coach supports participants in achieving their personal goals through individualized care plans. This role collaborates closely with participants, their families, and support teams to foster a person-centered approach, ensuring that each participant's needs and preferences are at the forefront of their care.

ESSENTIAL FUNCTIONS

  • Directly assists assigned participant in accordance with their Plan of Care, ensuring that all activities are aligned with their goals and preferences
  • Work collaboratively with the participant’s person-centered team to implement and monitor the effectiveness of the participant’s plan. Engage in regular discussions to assess progress and make necessary adjustments.
  • Demonstrate and promote person-centered thinking in all interactions, encouraging participants to express their preferences and make choices that reflect their individual needs
  • Provide training and guidance to participants, families, guardians, natural and unpaid supports, and team members to overcome barriers to success. Equip them with the tools and knowledge necessary to support the participant effectively.
  • Maintain accurate, confidential records of participant progress and interventions. Ensure that all documentation is submitted in a timely manner, adhering to organizational policies and legal requirements.
Requirements:

SUPERVISORY RESPONSIBILITY

No supervisory responsibilities of employees or contractors

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS

  • Primary activities are conducted within a well-lit, climate-controlled locations.
  • Ability to sit for extended periods and possess the normal range of body motion.

EDUCATION AND/OR EXPERIENCE

  • High school diploma or GED
  • Two years of experience in the field of intellectual or developmental disabilities or completed twelve hours of college coursework in human services field;
  • Completes all KY State training requirements

OTHER REQUIREMENTS

  • A clear state police criminal record check is required for any state in which the person has resided in the past year. If any prior convictions for offenses outlined in KRS Chapter 17.165 (1)-(3) or a prior felony conviction, employment work will not be offered. Additionally, annual background checks may be required.
  • Understand and comply with the mission and values of a VersiCare Group company.
