What You'll Do
As the Hospice Nurse Liaison, you'll play a key role in connecting patients, families, hospital providers, and the Granite VNA hospice team by:
- Building and maintaining strong relationships with physicians, nurses, case managers, social workers, and other key contacts at a local hospital.
- Serving as the on-site ambassador for Granite VNA's hospice program and promoting awareness of our services.
- Guiding referrals by facilitating the hospice intake process and transferring essential referral information from the hospital's electronic medical record (EMR) to our Intake team.
- Maintaining a visible presence throughout the hospital and serving as the go-to resource for hospice referrals and questions.
- Assessing patients and reviewing medical records to determine hospice appropriateness while conducting hospice informational visits and General Inpatient (GIP) hospice admissions as needed.
- Educating patients and families about hospice philosophy, eligibility, and goals of care with empathy and compassion.
- Partnering closely with the Palliative Care Team to identify patients who may benefit from hospice services.
- Ensuring seamless communication between hospital staff, hospice clinicians, facilities, and community partners.
- Collaborating with the General Inpatient (GIP) Nurse to provide responsive service, coordinate smooth transitions, and ensure continuity of care.
- Completing accurate and timely documentation.
- Monitoring hospital partner satisfaction and working with the Director of Community Engagement to strengthen relationships and improve service.
- Using critical thinking to navigate complex cases, educate referral sources, and identify opportunities for process and quality improvement.
- Coordinating with vendors to arrange durable medical equipment and infusion therapies when needed.

- A nurse who has at least 6 months of hospice experience

Additional Responsibilities

- Travel to assigned work locations as needed.
- Follow all safety policies and procedures to protect yourself and others.
This is a full-time position with a schedule generally occurring: Monday–Friday | 8:00 a.m. – 4:30 p.m.
Hours may vary based on the needs of the agency.

Requirements:- Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC)
- At least 6 months of hospice related experience preferred
- Previous experience in business development or care coordination
