Case Manager - Young Adult Services Street Outreach in Plano, Texas at City House Inc

General Purpose:

The Young Adult Services (YAS) Case Manager is responsible for the development and implementation of safe and affirming programs, services, and community resources and referrals to youth 16-22 who are experiencing homelessness. This is done through assisting with the function of the Street Outreach Program within the Youth Resource Center and through Roving.

Essential Functions:

? Provide intensive, trauma-informed case management and crisis intervention to youth experiencing homelessness. This is on a case-by-case basis with Street Outreach clients.

? Responsible for maintaining daily operations of the Youth Resource Center including opening, closing, overall cleanliness, overseeing outreach supplies and providing clients with resources, referrals, and services either within or outside of the agency.

? Assist with roving within the community to locate and connect with potential clients experiencing homelessness.

? Responsible for the development and implementation of safe and affirming programs and services for LGBTQ+ youth at-risk of or currently experiencing homelessness.

? Work in harmony with the YAS team to determine and review clients who apply for services and make relevant referrals as needed.

? Assist in application process for transitional living program and collaborate with YAS team.

? Partner with the client to develop and implement a client-centered case plan including housing & basic needs, mental health & safety plans, as well as goals to obtain housing stability.

? Assist clients in identifying barriers to independent living and provide information and assistance necessary to overcome such barriers (i.e., affordable housing options, educational development, job readiness, life skills development, etc.).

? Coordinate individual mental health services, substance use services, and other services identified by clients as opportunities for growth.

? Maintain appropriate documentation as required by the agency and funders.

? Collect, enter, and create reports using information entered; database and shared drive systems to produce outcomes regarding client’s goals, grantee required statistics, for other statistical information as requested. Manage HMIS, enrolling youth into CAS, and monthly reporting.

? Have a working knowledge of trauma-informed care, TBRI, motivational interviewing, positive youth development, low barrier client services meeting the client where they are emotionally.

? Participate in staff meetings, case reviews, and on-call rotation.

? Maintain full confidentiality regarding clients/residents and staff always during work and non-work hours.

? Employees may also be asked to perform other reasonably related business duties for the overall efficient operation of the organization and employees will adhere to all policies of City House Inc.

Qualifications/Basic Job Requirements:

? Bachelor's degree in social work or related social field required. Masters preferred

? Ability to be flexible in meeting program needs.

? The ability to effectively work and communicate (oral and written) with individuals from culturally diverse backgrounds.

? Strong organization and time management skills and ability to multitask in a fast-paced and changing environment.

? Proficiency in basic technology skills.

? Minimum of one year of experience working with at-risk and/or homeless youth or young adults or related experience.

? Knowledge of issues related to youth/young adult homelessness, abuse and neglect, life skills, child and adolescent development.

? Report any suspected abuse in a timely fashion.

? Valid driver's license and auto insurance.

? CPR certification must be current or obtained within 30 days of hire. Willingness to take first aid training and de-escalation training.

? Ability to establish and maintain effective work relationships with other employees and the public.

? This position is funded through state and federal grants and is subject to termination if funding ceases.
