Job Description
JOB SUMMARY:
Provides direct patient care to patients under the supervision of RN and documents care on electronic medical record. Performs reception duties and clerical tasks. Assists in ER with greeting, rooming, and registration of patients.
Assists RN’s/LPN’s in completing care plan tasks to assist patients in having an optimal outcome.
- Demonstrates knowledge of equipment, policies and procedures and supplies necessary to provide direct patient care
- Demonstrates aseptic and hygiene techniques to prevent the spread of infection to patient, self and others
- Provides assistance to the patient with ADLS including baths, ambulation, oral cares etc..
Uses interactive communication skills with patients and families, peers and providers to promote, evaluate and direct patient care and relationships with others.
- Communicates through clear, concise, accurate electronic documentation following policy and procedures.
- Demonstrates effective communication skills to assist patient care team in overall plan of care
- Contributes to building a positive environment through display of positive behaviors and the use of first party communication
- Recognizes and clearly understands that each individual is responsible for his/her own actions.
- Is accountable for his/her own actions.
Maintains and promotes quality of care that is safe, reliable and patient centered.
- Evaluates the quality and effectiveness of current plan of care and communicates changes in patient with RN.
- Provides for safe patient care transitions and hand offs through effective communication with patient/family and RN.
- Involves patients' families in patient care
- Assists RN in completing hourly rounding
- Uses safety equipment/supplies as needed. i.e. gait belts, patient lifts, skin prevention, fall prevention.
- Promotes and ensures a safe environment in patient room and care areas (patient has call light within reach, fall prevention alarms on, and etc.)
- Uses proper hand hygiene practices and follows isolation procedures.
- Can use appropriate lifting, and repositioning techniques when moving patients and uses patients lifts per policy and procedure.
Uses customer service skills in all interactions with patients and families.
- Greets, assists patients/families, hospital staff and visitors in a courteous manner. Assists them as needed
- Greets and assists patients/families in ER and assists them to a room
- Introduces self to patient/families and writes name on board when providing direct patient care.
Maintains patient medical records, registers patients and correctly reviews patient charge slips to ensure accurate patient information.
- Files patient information in correct record.
- Creates medical record for new patients and tears down charts for those discharged
- Puts patient records in appropriate order, including ER records.
- Completes charge slips accurately, returns to nurse if necessary, each shift and sends to correct department
- Obtains previous medical records for providers or staff when requested
Promotes teamwork by positive attitude and putting patient needs first.
- Provides peer feedback in constructive manner
- Engages in direct one-to-one communication and promotes teamwork

- Actively participates in peer review processes
- Reports issues to supervisor if unable to resolve conflict with peers
Completes other clerical duties as assigned.
- Ensures transfers, discharges and accommodation codes are entered correctly
- Completes all admission paper work as necessary, copies insurance cards and collects all information necessary for billing purposes.

EDUCATION/EXPERIENCE:
High school diploma or equivalent
REQUIRED CREDENTIALS (License, certification, registration):
Certification as nursing assistant is preferred, BLS certification required. 1 year of healthcare experience is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
Must have good interpersonal skills; must have knowledge of medical terminology. Function under high stress situations. Basic computer skills and knowledge. Requires ability to do detailed work, have strong communication and customer service skills.
EMPLOYMENT VARIABLES:
This position is full time 32-36 hours per week (.8-.9 FTE) and will work (7am-7pm) Friday-Sunday, and rotating holidays.
The nursing department is open 24 hours/day. These needs can change dependent upon staffing availability. May be required to work overtime, take low census on-call.
