Job Description
Position Summary:
A Population Health Manager is responsible for overseeing and implementing strategies that improve health outcomes for specific populations. The role works with healthcare providers, community organizations, and internal and external stakeholders to develop programs that address social determinants of health, reduce disparities, and enhance the quality of care.
Seeking an individual who is specialized in quality improvement workflows, patient engagement, population health initiatives, and Epic EHR documentation. This role will be integral in educating and guiding clinical care coordination across the organization to ensure adherence to best practices that improve patient care and quality performance
Key Responsibilities
Alongside Director of Quality Improvement and Population Health the key responsibilities for this role are;
• Program Development & Implementation: Design and implement initiatives to improve health outcomes and manage chronic diseases.
• Data Analysis & Reporting: Use data analytics to identify trends, assess risks, and measure the effectiveness of interventions.
• Interdisciplinary Collaboration: Work with care team members and population health coordinators to coordinate care and resources.
• Patient & Community Engagement: Develop outreach programs that educate and engage patients in preventive care and wellness activities.
• Regulatory Compliance: Ensure programs align with healthcare policies, value-based care models, and accreditation requirements.
• Quality Improvement: Identify gaps in care and implement evidence-based practices to improve patient outcomes.
Documentation and Reporting
· Ensure compliance with all regulatory and organizational documentation standards in Epic EHR.
· Assist in the creation of user-friendly guides and resources for Epic documentation and workflow processes.
· Ensure all population health activities comply with relevant regulations, policies, and accreditation standards.
· Document patient interactions, care coordination efforts, and outcomes in the electronic health record (EHR) system.

Requirements:Required Skills & Qualifications
- • Education: Bachelor’s or Master’s in Public Health, Nursing, Healthcare Administration, or a related field.
- • Experience: Background in healthcare management, population health, or quality improvement.
- • Skills: Strong analytical, communication, leadership, and project management abilities.
- • Knowledge: Familiarity with healthcare regulations, data analytics tools, and population health strategies.
- · 3+ years of experience in a clinical setting with experience in quality improvement initiatives.
- · Proficient in Epic EHR and other clinical software systems (preferred)
- · Strong communication, teaching, and mentoring skills.
- · Ability to work effectively within an interdisciplinary team.
Skills and Competencies
- · In-depth knowledge of healthcare quality improvement principles, including performance measurement and population health management.
- · Expertise in Epic EHR workflows, with the ability to train others on system use and documentation standards.
- · Optimal use of Microsoft Office Suite
- · Excellent interpersonal skills, with a focus on patient engagement and team collaboration.
- · Strong problem-solving abilities, particularly in workflow optimization and process improvement.
