Regional Nurse Coordinator – FULL-TIME

Home-based - -NY/NJ | Monday–Friday, 8:30 AM–4:30 PM

ABOUT THE JOB

The Franciscan Province of Our Lady of Guadalupe is seeking a dependable, caring and detail-oriented Regional Nurse Coordinator supporting retired Franciscan friars in the Northeast states. Regional travel involving use of a company vehicle.

THE IDEAL CANDIDATE

A Registered Nurse who has a passion for working with older adults, likes working independently and takes pride in their work. A kind, compassionate and respectful professional who provides care to religious communities in the Northeast region of the United States.

KEY RESPONSIBILITIES

  • A traveling resource facilitating access to healthcare resources and medical care to a dispersed population of retired religious men residing in commercial and fraternal settings….assist friars in maintaining optimal level of wellness.
  • Complete in-person, physical and mental health assessment of all friars 70 years and older or friars with significant physical or mental health needs who reside in designated area.
  • Provides direct care to friars as required.
  • Performs all duties and activities in compliance with professional standards of practice, application of legal requirements and ethical guidelines and Aging, Health & Wellness team guidelines.
  • Communicate proactively with leader of AH&W and the local Friar leader (Guardian) to planfully meet the healthcare needs of the retired friars

QUALIFICATIONS & ATTRIBUTES

  • BS degree in nursing with 3-5 years of medical/surgical care coupled with 2+ year’s experience with home care. Experience with geriatric and/or dementia care a plus.
  • Sensitive and collaborative work style with exceptional verbal and written communication style.
  • Inquisitive and service-oriented mindset coupled with desire to make the world a better place and maximize the effectiveness of people in a faith-based organization.
  • Focus on patient-centered care and uses collaborative approach with patients.
  • Self-directed work style.
  • Ability to use TEAMS, ZOOM, and maintain electronic medical records.
  • Experience with case management, eldercare and home care activities
  • Proven experience developing and nurturing relationships
  • Ability to lift, push and pull up to 40 pounds and bend, kneel and reach throughout the day.
