About the Role
The Engagement Specialist serves as a key relationship-builder and community presence within Matter Health’s care model. This role is responsible for engaging older adults living within assigned communities, helping patients navigate their healthcare journey, and supporting ongoing patient connection, outreach, enrollment, and retention efforts.
This is a highly interactive, field-based position ideal for someone who enjoys building trusted relationships, connecting with people face-to-face, and making a meaningful impact within underserved communities. Engagement Specialists work closely with operational and clinical teams to help ensure patients remain engaged, supported, and connected to care.
The ideal candidate is compassionate, outgoing, organized, and comfortable working in a fast-paced environment where relationship-building, communication, and community engagement are at the center of the role.
What You’ll Do
- Build trusted relationships with older adult patients and community members
- Maintain a visible and consistent presence within assigned communities
- Support patients throughout their care journey, including outreach, scheduling, enrollment, and follow-up
- Educate patients on Matter Health’s care model and help guide them through available services
- Conduct outreach through in-person visits, phone calls, and community events
- Partner closely with clinical and operational teams to support ongoing patient engagement and retention
- Help identify barriers to care and support patients with empathy, compassion, and urgency
- Track outreach activity, engagement efforts, and patient interactions within Salesforce and internal systems
- Serve as a trusted community resource and advocate for patients
- Assist with building awareness and engagement within local communities and housing sites
What We’re Looking For
- 1–2 years of experience in community outreach, healthcare, customer service, hospitality, sales, or other relationship-focused roles
- Strong communication, relationship-building, and interpersonal skills
- Compassion for seniors and underserved populations
- Organized, proactive, and comfortable working in a fast-moving environment
- Ability to work independently while collaborating closely with cross-functional teams
- Comfortable working in community-based and field environments
- Valid driver’s license and reliable transportation required
- Experience with Salesforce, healthcare outreach, Medicare populations, or community engagement is a plus
- Bilingual Spanish is a plus
