Job Description
East Arkansas Family Health Center is seeking a compassionate and knowledgeable Nutritionist to join our healthcare team. This position promotes health and wellness through evidence-based nutrition education, counseling, and disease prevention. The Nutritionist works closely with physicians and interdisciplinary care teams to develop individualized nutrition plans, monitor patient progress, and support healthy lifestyle changes.
In addition to providing direct patient care, the Nutritionist plays an active role in community outreach, health education, and wellness initiatives designed to improve the overall health of the communities we serve.

- Design and implement individualized therapeutic nutrition care plans based on physician diagnoses and evidence-based nutrition standards.
- Educate and counsel patients, families, and caregivers on healthy eating habits, disease prevention, and dietary modifications.
- Develop nutrition education materials and provide monthly wellness resources for patients and staff.
- Conduct individual and group nutrition education sessions to promote healthy lifestyle choices.
- Provide nutrition consultations in person, by telephone, or through approved outreach or home visits when appropriate.
- Monitor patient progress and modify nutrition plans as needed to achieve desired health outcomes.
- Maintain accurate documentation and complete monthly and quarterly productivity reports.
- Collaborate with physicians, nurses, care coordinators, and other healthcare professionals to support comprehensive patient care.
- Participate in community health fairs, outreach events, and wellness initiatives to promote nutrition education.
- Travel to off-site clinic locations at least once per month to provide nutrition counseling and patient education.
- Perform other duties as assigned.
- Bachelor's degree in Food & Nutrition, Nutrition Science, Food Management, Dietetics, or a related field required.
- Experience in nutrition counseling, community health, outpatient care, or a healthcare setting preferred.
- Strong knowledge of nutrition principles, health promotion, and disease prevention.
- Excellent communication, patient education, and counseling skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Strong organizational, documentation, and time management skills.
- Passion for community health, wellness, and improving patient outcomes.
This position is primarily clinic-based to support direct patient care and collaboration with the healthcare team. However, limited remote work may be approved for eligible duties such as virtual nutrition counseling, documentation, patient follow-up, and administrative responsibilities, based on operational needs and supervisor approval.
