Scheurer Health-Harbor Beach is seeking a full-time Community Health Worker (CHW) to join our team.

Responsibilities

• Provide outreach, care coordination, resource navigation, and health education to support students and their families
• Identify and address barriers that my impact students and families
• Connect individuals to appropriate healthcare, behavioral health, social services, and other community partners

Requirements

• High School Diploma or G.E.D Equivalent
• Two years of experience in a healthcare-related field
• For the school-based clinic, experience working with children, adolescents, families, and schools
• Community Health Worker certification, if not currently certified, you must certify within six months of hire
• The CHW will work in both Scheurer Physician Clinics and School Based Clinics
• Be able to interact with all age groups
