What You’ll Do:
- Provide weekend on-call coverage and respond promptly to patient and caregiver phone calls, medical or emotional crises, and patient deaths
- Conduct comprehensive nursing assessments and complete admissions as needed
- Make home or facility visits to address urgent patient care needs and provide direct care when appropriate
- Communicate effectively with attending physicians, GSH staff, and other agencies to coordinate optimal patient care and resources
- Educate and support patients and families regarding hospice philosophy, disease progression, symptom management, stages of dying, and the plan of care
- Collaborate with the Interdisciplinary Team (IDT) by reporting changes in patient status and communicating important patient and caregiver concerns
- Assist RN Case Managers with scheduled after-hours visits and patient care needs to ensure continuity of care
- Complete timely and accurate documentation of all patient interactions, visits, assessments, and care provided in the electronic medical record
- Provide a thorough report to the Interdisciplinary Team following each shift
- Maintain clinical competencies and participate in required staff meetings, in-services, and continuing education
What We’re Looking For:
- Clinical Expertise – Graduate of an accredited nursing program with current Virginia RN licensure
- Experience – Minimum 2 years in nursing, including hospice, home health, med/surg, acute care, or geriatric nursing
- Critical Thinking & Decision-Making – Strong judgment, problem-solving, and ability to handle complex patient situations
- Communication Skills – Excellent verbal, written, and interpersonal skills; able to work collaboratively with the IDT
- Technology Skills – Experience with EMR documentation and general computer applications
- Compassion & Professionalism – Provides patient-centered care with empathy, discretion, and integrity
Schedule
Schedule: Weekend Coverage - Saturday - Sunday 8:00am - 8:00pm
Location: New River Valley Service Area
Starting Salary: $36/hour
This is the starting salary for the position. Compensation may vary within the established salary range based on relevant experience, education, licensure, certifications, and other qualifications.
