COMPENSATION & LOCATION
The compensation range is $28 - $29 per hour.
This role is at our Elk Grove Village location, with a Hybrid schedule 2–3 days per week onsite.
POSITION SUMMARY
The Sr. Patient Care Coordinator (III) serves as a frontline operational leader specializing in specialty project execution, new therapy launches, and operational recovery across the Rare Disease COE therapy programs. This role leads complex issue resolution, drives targeted interventions to restore or improve operational performance, and ensures continuity of care for patients on high-touch or high-risk therapies. Functioning as a subject matter expert, the Sr. PCC models exceptional patient-centered communication, rigorous documentation, and compliance-aligned best practices. The role partners closely with pharmacists, operational leaders, and cross-functional teams to identify root causes, implement corrective actions, and sustain high-reliability performance. The Sr. PCC owns specialty initiative execution — including pilot programs, new therapy launches, workflow redesign, technology optimization, and accreditation support projects — while providing peer coaching to elevate team capability and consistency.
ESSENTIAL JOB DUTIES:
- Specialty Projects & New Therapy Launches: Lead or co-lead the operational execution of new therapy launches, pilot programs, and manufacturer program implementations. Develop and refine launch workflows, documentation standards, and training materials. Serve as the primary frontline point of contact for launch readiness activities and post-launch stabilization.
- Operational Recovery & Performance: Lead or coordinate targeted recovery initiatives (backlog resolution, throughput optimization, rapid stabilization). Maintain real-time visibility of backlogs and queues; communicate risks and recovery progress to leadership. Use data (including Excel dashboards) to drive prioritization and validate recovery outcomes.
- Patient-Centered Care & Safety: Serve as a senior escalation resource for complex or high-risk patient scenarios; ensure safe, timely continuity of therapy. Anticipate adherence risks and collaborate with pharmacists on mitigation plans. Model exemplary patient communication and safety compliance.
- Medication Management & Specialty Operations: Oversee and prioritize high-complexity workflows (urgent starts, time-sensitive biologics) to meet service-level expectations. Identify systemic issues impacting turnaround times or accuracy; recommend operational fixes. Ensure audit-ready documentation and data integrity across systems.
- Quality & Compliance: Identify root causes of recurring quality and compliance issues; support corrective and preventive actions. Reinforce adherence to SOPs and accreditation standards; participate in launch and ongoing readiness activities.
- Leadership, Competency & Team Support: Provide peer coaching, role modeling, and day-to-day leadership support in the absence of a supervisor/manager. Drive cross-training and multi-therapy competency to ensure operational flexibility during surges and launches. Foster a culture of accountability, continuous improvement, and patient-first service.
PERFORMANCE EXPECTATIONS:
Specialty Projects & Launch Execution
- New therapy launches and specialty projects executed on time and in alignment with operational readiness standards.
- Launch workflows, documentation, and training materials developed and validated prior to go-live.
- Post-launch stabilization achieved within defined timeframes; issues escalated proactively.
Operational Recovery & Throughput
- Backlog reduction and throughput targets achieved during recovery initiatives.
- Real-time queue visibility maintained and communicated to leadership proactively.
- Data-validated recovery outcomes reported to Manager, Therapy Operations.
Patient Care & Safety
- Complex patient escalations resolved safely and within defined timeframes.
- Therapy continuity maintained for all high-touch and high-risk patient cases.
Quality & Compliance
- Audit-ready documentation maintained; SOP adherence consistently demonstrated.
- Root cause identification and corrective action support contributions measurable.
Team Leadership & Development
- Peer coaching and cross-training contributions documented and recognized.
- Leadership continuity provided effectively in absence of supervisor/manager.
Education
- High School Diploma or Equivalent (or equivalent 3-5 years experience)
- Preferred: Associate's or Bachelor's Degree (or equivalent 5+ years experience)
Certifications (required)
- Active State Pharmacy Technician License as required by state.
Required Knowledge, Skills & Training :
- Advanced knowledge of specialty pharmacy operations, new therapy launch workflows, accreditation-aligned documentation, and operational recovery practices.
- Demonstrated experience leading or supporting specialty project execution, workflow redesign, or new program implementations in a specialty pharmacy setting.
- Strong analytical and prioritization skills with the ability to lead recovery efforts and manage competing project timelines.
- Proficiency with pharmacy systems; intermediate-to-advanced Excel skills (pivot tables, lookups, trend tracking, dashboards).
- Proven experience supporting operational recovery, backlog reduction, and service-level restoration.
- Strong communication and facilitation skills; able to coordinate across pharmacists, operations leaders, and cross-functional teams.
- 2+ years of hands-on experience with CPR+, Caretend, or SPRx specialty pharmacy platforms strongly preferred.
- 3–5+ years of experience in specialty pharmacy, patient support services, or healthcare operations with demonstrated escalation ownership.
Preferred Knowledge, Skills & Training
- 5+ years in specialty pharmacy or hub services with multi-therapy responsibility.
- Experience mentoring peers, acting as team lead, or covering supervisor/manager in their absence.
- Demonstrated familiarity with Joint Commission/URAC standards and audit readiness activities.
- Experience supporting or leading new manufacturer program launches or REMS program implementations.
