We are seeking a compassionate and dedicated Social Worker to join our team. In this role, you will assess client needs, develop care plans, connect individuals with community resources, and provide ongoing support to help improve outcomes.

Responsibilities:
Conduct client assessments to identify needs and concerns.
Develop and implement individualized service plans.
Provide counseling, support, and resource referrals.
Coordinate with healthcare providers, case managers, and community agencies.
Maintain accurate case documentation and follow-up records.
Monitor client progress and adjust plans as needed.
Advocate for clients and ensure access to appropriate services.

Requirements:
Experience working in a hospital environment.
Bachelor’s degree in Social Work or related field.
State licensure or eligibility for licensure.
Strong communication, organization, and interpersonal skills.
Ability to work independently and as part of a team.
Experience in case management, counseling, or social services preferred.
