Job Summary:
Under the general supervision of the Center Manager, the Transportation Coordinator plans, organizes, and implements the transportation services of the Jordan Valley Senior Care program. May occasionally serve as a Driver depending on staffing needs.
Key Responsibilities:
- Plan and coordinate all scheduling activities related to transportation for Jordan Valley Health Senior Care participants. Work with Interdisciplinary Team, participants, and family members to ensure pick-up and delivery of participants are within acceptable time frames.
- Supervise the activities of the Drivers daily. Receive reports regarding participant status and communicate concerns to Interdisciplinary Team.
- Ensure drive times and participants’ time in vehicles during transit are within regulatory requirements.
- Oversee competency requirements and training of Drivers, including body mechanics and use of mechanical assistance to transport participants.
- Occasionally serve as Driver depending on staffing needs and be up to date with Driver competencies and licensure/insurance requirements.
- Coordinate all outside service provider transportation arrangements, including proper vehicle maintenance and condition.
- Assure all necessary program records are maintained according to policy.
- Participate as a member of the Interdisciplinary Team: attend staff meetings, take part in participant care planning, and communicate any reported changes in participant condition to team members.

Requirements:Required Qualifications:
- Minimum 2 years supervisory experience.
- Minimum 1 year experience transporting individuals with special mobility needs.
- Valid driver's license.
- BLS Certification required within 90 days of hire.
- Defensive driving course required to be taken within 90 days of hire.
- First aid course required to be taken within 90 days of hire
Preferred Qualifications:
- High school diploma or equivalent.
