Job Title: Residential Case Manager

Reports to: Girlstown Clinical Supervisor
FLSA Status: Full Time Exempt
Job purpose
Girlstown Residential Program serves female children (ages 10-17). The residents of Girlstown are in foster care, but due to their behavioral and/or emotional needs as a result of the trauma they have suffered, are not successful in a traditional foster home. This setting allows for them to develop coping strategies so that they can be fully productive members of the community.
The Residential Case Manager position is responsible for the provision and/or coordination of all client services indicated to achieve the identified permanency plan.
Duties and responsibilities

- Responsible for coordinating all case management services
- Monitor and report on client progress
- Reviews all information and meets with the client, birth family, significant others and referring worker to develop treatment plans
- Utilization of the required online database
- Coordination of emergency psychiatric hospitalizations
- Work with consulting Psychiatrist on treatment planning & medication monitoring
- Complete Action Summaries & progress reports as assigned by the Girlstown Program Manager in a timely manner.
- Complete intake interviews, intake assessments and intake paperwork as assigned.
- Collect and obtain client specific information to assist in permanency planning
- Reviews all information and meets with child, birth family, significant others and assigned state worker to develop treatment plans in a timely manner
- Maintain client files by filing, updating information, etc. in accordance with Licensing & Contract Compliance standards
- Schedule and facilitate ongoing progress review meetings and case conferences to assist client/family in meeting goals and objectives of the case plan
- Responsible for maintaining contact with assigned state worker and parents/guardians (if applicable)
- Provides all case management services to client and family
- Attend school related conferences when needed
- Transport clients as necessary to link services for client and family
- Assume court responsibility as assigned per contract and complete court reports and dispositions in a timely manner
- Inform immediate supervisor verbally and in writing any issues that impact the clients, employees & administration of the agency.
- Inform administration immediately of any client abuse or neglect.
- Encourages and monitors clients’ attendance to enhance clients’ ability to complete educational program.
- Records and reports all client incidents/behavioral problems at school and monitors/intervenes in an appropriate manner to assist clients in solving school related issues.
- Evaluate program strengths/deficits and make recommendations in order to improve service delivery.
- Continually monitor, interact with and counsel clients within the school to assess behavioral problems and deescalate problematic situations.
- Inform administration immediately of any client abuse or neglect.
- Enroll all clients in appropriate accredited school program within (5) days of admission, providing information as necessary; documenting same, in order to meet DHHS licensing & contractual standards.
- Schedule/assist clients in selecting appropriate/required classes in order to meet Promotion and Graduation Requirements (PGR)
- Seek and obtain financial assistance for clients desiring to pursue higher education/participate in extra-curricular activities to achieve education objectives.
- Meet with Clients’ teachers to intervene or assist in completing tasks
- Provide on-call crisis availability to residents on emergency phone.
- Be willing to drive to off site locations and at times, long distances to serve clients to the best of our ability.
Skills and Qualifications
- Bachelors Degree in a relevant field of human services: Social work, Psychology, Counseling, Child development, Criminal justice or similar field required.
- Knowledge and experience in working with abuse/neglect teenagers preferred
- Understanding of working with children who have experienced trauma
- Must be trained in First Aid and Cardiopulmonary Resuscitation (CPR).
- Commitment to the goals and philosophy of the organization including cultural competency objectives.
- Must be highly motivated and flexible
- Must display sophisticated writing and interpersonal communication skills.
- Excellent organizational skills and ability to work independently or with a team.
- Strong skills in Microsoft Office including Word, Excel, Outlook, etc.
Working conditions
- This job operates in a clerical office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
- Responsible to work with On-Call when there is an emergency and perform On-Call duties as needed.
- May be required to work nights and Weekends and have a flexible schedule.
- This position requires travel; Must have reliable transportation and maintain a good driving record.
Physical requirements
- The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; and extensive reading.
- Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
- Must be able to perform physical behavior management techniques safely.
- Must be able to make trips up and down stairs during the work day
- Positions in this class typically require fingering, grasping, talking, hearing, seeing and repetitive motions.
- May be exposed to hazardous materials found in a home or general office environment.
- Use of household cleaning products and disinfectants may be required.
