- Start of Care: 2.2 - 2.5 Points per Visit
- Discharge: 1.0 - 1.5 Points per Visit
- Resumption of Care / Recertification: 1.5 - 1.75 Points per Visit
- Revisit: 1.0 Point per Visit

- Provide one-on-one skilled nursing care and treatment to patients in their homes.
- Assist registered nurses in assessing patients' conditions and contributing to care planning.
- Demonstrate expertise in home health and discipline-specific patient care skills.
- Perform comprehensive patient assessments and collaborate with multidisciplinary care teams.
- Monitor vital signs, administer treatments, and educate patients and families on medical conditions.
- Manage patient episodes according to protocol.
- Complete documentation accurately and timely.
- Current and unencumbered LPN license
- 1+ year(s) of Certified Home Health experience as an LPN
- Current CPR certification
- Comfortable with technical tools and EMR for visit documentation
- Reliable transportation, driver's license, and car insurance
- Proof of Immunization/Exemption for Flu and COVID-19
- TB Test or Chest X-Ray
