Join a mission-driven team where your nursing care creates meaningful impact every day — helping patients remain safe, comfortable, and cared for at home.

If you’re looking to leave the fast pace of inpatient care for a role with greater autonomy, flexibility, and connection, this is your opportunity to make a difference where it matters most.

  • Granite VNA embroidered apparel stipend
  • The chance to care for patients while enjoying the beauty of New Hampshire’s lakes, back roads, and communities
About the Role

The Hospice Community RN provides skilled, compassionate nursing care to patients in home and facility settings. Working closely with patients, caregivers, physicians, and the interdisciplinary team, you will assess needs, develop care plans, provide education, coordinate services, and ensure patients receive exceptional hospice care aligned with best nursing practices and NH Board of Nursing standards.

What You’ll Do
  • Develop and manage individualized hospice care plans
  • Provide patient and family education and support
  • Collaborate with physicians and interdisciplinary team members
  • Document assessments, interventions, and patient progress
  • Supervise LPNs and LNAs assigned to patient care
  • Mentor new nurses and students
