The Hospice Community RN provides skilled, compassionate nursing care to patients in home and facility settings. Working closely with patients, caregivers, physicians, and the interdisciplinary team, you will assess needs, develop care plans, provide education, coordinate services, and ensure patients receive exceptional hospice care aligned with best nursing practices and NH Board of Nursing standards.
What You’ll Do- Develop and manage individualized hospice care plans
- Provide patient and family education and support
- Collaborate with physicians and interdisciplinary team members
- Document assessments, interventions, and patient progress
- Supervise LPNs and LNAs assigned to patient care
- Mentor new nurses and students
- Travel throughout the Granite VNA service area for patient visits

Requirements:Minimum Qualifications and Requirements
- Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
- One to two years of recent general medical/surgical or skilled/acute care nursing experience.
