Nurse Navigator - Geriatric Emergency Department

POSITION STANDARDS
-
- Graduate from an accredited Nursing Program required. Bachelor of Science Degree in Nursing (BSN) preferred with at least 1 yearof experience working with geriatric population.
- Associate Degree (ADN) prepared nurses with 2 years' experience.
- Strong problem solving skills with demonstrated skills to defi ne scope and processes.
- Strong interpersonal, organizational, and communication skills; ability to work within a collaborative team environment; ability tocommunicate eff ectively and respectfully to a diverse community.
- Must be skilled in the use of MS Word, Excel, PowerPoint and Outlook.
- Registered Nurse in the State of Vermont and New Hampshire.

PART TWO: FUNCTIONAL RESPONSIBILITY

Position Objective
-
Provides remote telephonic and virtual “off -hours” care navigation support to Emergency Department (ED) personnel throughoutthe Geriatric Emergency Department Network for high risk/high need older adults being cared for in
an ED.

Performance Expectation
- Responds to calls outside of normal business hours from ED personnel with concerns for high risk/high need older adults.
- Serves as a content and resource expert for interdisciplinary team members in management of geriatric patients.
- Keeps apprised of established and emerging evidence-based best practices in geriatric emergency care.
- Maintains an understanding of regulatory requirements and available resources for care and support of the geriatric patient.
- Collaborates with members of interdisciplinary team and community agencies to obtain, provide, and exchange information related to patient’s health care needs.
- Collaborates with ED personnel to establish a plan of care to hand-off to the onsite team the next day.
- Consults with the on-call geriatrician for complex cases.
- Promotes concordant care by asking ED personnel about patients’ Advance Care Plans.
- Communicates plan of care with project coordinator and ED team onsite the next day to ensure continuity.
- Tracks calls, cases, interventions and outcomes for reporting purposes and quality improvement.
- Identifies gaps and opportunities; conveys to project leadership.
- Performs other duties as required or assigned.
