Position Summary:

The RN Case Manager/Delegating Nurse (RN CM/DN) is responsible for the ongoing care of the clients that are assigned to them according to the guidelines set forth by the Developmental Disabilities Administration, Maryland Board of Nursing, and outlined in the DHCA Medication and Nursing Related Policies and Procedures manual. This responsibility also includes traveling to our customers for the purpose of the delegation of and oversight of approved functions to agency staff that work at the client’s residence. The RN CM/DN is designated as a Mandatory/Essential employee. This employee is a Registered Nurse providing essential care to intellectually and developmentally disabled persons in the community and are required to report to work during State and/or National emergency periods. This may include traveling during periods with domestic travel restrictions.

Essential Responsibilities:

• Serve as the Delegating Nurse/Case Manager for each client assigned.

  1. When the client is first assigned, visit the client in the environment in which delegation occurs, conduct and complete an initial clinical assessment and develop the nursing plan of care.

  2. Every 45 days, visit clients in the environment in which delegation occurs for assessment.

  3. Conduct Interim visits within 48 hours (almost all visits outside the 45 day schedule - this is an opportunity for nurses to earn additional pay)

  4. Delegate administration of medication and other delegatable tasks to certified Medication Technicians.

  5. Train and ensure staff’s competency in nursing care plans, delegated nursing tasks, as well as managing medication errors.

Minimum Qualifications:

• Must have a working “smart” cell phone capable of making calls, receiving calls, receiving emails and sending emails.
• Current valid compact state of Maryland Registered Nurse license.
