About This Position
The Oncology Lay Navigator position is a grant funded position with Alliance for Equity in Cancer.
Job Overview:
The Patient Navigator collaborates with the primary care provider, all members of the health care team, and community organizations to accept referrals and manage cancer screening/oncology caseload. She/he works with other healthcare professionals to establish and maintain a climate of mutual respect, dignity, diversity, ethical integrity, and trust.
Qualifications:
Required:
- High School Diploma, required
- The candidate must possess a valid driver's license and be willing and able to drive a car.
- Ability to effectively communicate preventive health and health management philosophies through community outreach
- Must have strong interpersonal skills and the ability to work with a vast array of internal and external health care providers.
- Demonstrates ability to be confident, personable, self-motivated.
- Demonstrates excellent communication skills, knowledge, judgment and leadership in a multidisciplinary environment.
- Knowledge of the relevant of health information and appropriate documentation methods. All patient related hospital departments, patients, families, visitors, nursing personnel and medical staff.
Preferred:
- Minimum of 2 years of health care in an office/clinic/hospital setting or human services experience preferred
- Chinese -bilingual in Mandarin or Cantonese preferred
- Haitian -bilingual in Haitian Creole, or French or Frespanol preferred
- Previous experience in presenting health education materials to individuals and small groups.
- Community Health Worker based care with a focus on culturally sensitive and linguistically appropriate assessment experience preferred
Essential Functions
- The Oncology Lay Navigator supports and executes the interventions and strategies in a particular territory to impact coordination of cancer care.
- Integrates with primary care and other specialty care, patient-centered communication and engagement in care and psychosocial and other supportive care with the goal to improve access to high-quality cancer care for underserved populations.
- The Oncology Lay Navigator supports care that is culturally sensitive and linguistically appropriate, building collaborations with trusted community-based partner organizations that can help address the social determinants of health and the innovative use of digital tools to reduce barriers to care.
