Job Description

Forensic Supportive Housing Specialist

Location: Glen Cove (2 days/week) & Baldwin (3 days/week), NY

Travel: Regular community-based travel throughout Nassau County

Schedule: Full-Time | Monday-Friday, 9:00 AM-5:00 PM

Flexible schedule required to meet program needs, including participation in an on-call rotation.

  • Non-Bachelor's Degree: $24.72/hour
  • Bachelor's Degree or Higher: $27.00/hour

In this role, you will work alongside individuals with behavioral health challenges and justice-involved backgrounds as they build meaningful lives in the community. Through advocacy, skill-building, care coordination, and supportive services, you will help residents maintain housing stability, achieve personal goals, and strengthen their connection to the communities where they live.

What You'll Do

As a Forensic Supportive Housing Specialist, you will:

  • Provide supportive housing services in accordance with OMH regulations and program standards
  • Help residents develop independent living and life-management skills
  • Support individuals with housing stability, apartment maintenance, budgeting, and community engagement
  • Conduct assessments and identify areas where additional support and skill development are needed
  • Coordinate services with treatment providers, community agencies, landlords, and other key partners
  • Advocate for residents and assist them in navigating community resources
  • Maintain accurate, timely, and compliant documentation of services and resident progress
  • Provide crisis intervention, problem-solving, and supportive guidance when needed
  • Participate in an on-call rotation to support residents outside of regular business hours
  • Travel throughout Nassau County to meet residents in their homes and communities

Required Qualifications

  • Associate degree or equivalent college credits
  • Experience working with forensic populations and/or individuals with complex behavioral health needs
  • Strong written and verbal communication skills
  • Valid New York State driver's license in good standing with a clean driving record
  • Daily access to a reliable vehicle for work-related travel
  • Ability and willingness to participate in an on-call rotation

Preferred Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, or a related field
  • Experience in supportive housing, case management, care coordination, or community-based services
  • Familiarity with Electronic Health Record (EHR) systems
  • Proficiency with Microsoft Office applications

Work Environment

This position combines:

  • Community-based field work and home visits
