Job Description

JOB SUMMARY


To make a measurable and positive difference in the lives of children and their families, youth and young adults (ages 5-23), who are coping with a mental health or substance abuse diagnosis, as they transition to adulthood. To contribute to the advancement of the Care Coordination team through mentoring, facilitation of training, and provision of feedback. To ensure quality community-based care, coordinating services and support. Care Coordinators work with children and their families, youth, and young adults to design a wellness plan that is intended to help build the skills needed to improve health, promote wellness, attain personal/family goals and enhance the overall quality of life. Using a strengths-based and individualized approach Care Coordinators help develop skills for independence, leadership, confidence, and empower individuals and families to take ownership of their lives and achieve their goals.


QUALIFICATIONS AND EXPERIENCE

Education/Training: A Bachelor’s Degree in the areas of social work, psychology, sociology, or criminal justice required. Master’s Degree preferred.


Skills: Excellent verbal and written communication skills. Ability to organize work and manage time efficiently. Able to relate to young adults and set appropriate boundaries. Energetic, outgoing, positive attitude, self-starter, creative, and a team player.

Experience: At least one year of Care Coordination experience with Wraparound Milwaukee.


STANDARD REQUIREMENTS


1. Support the Mission, Core Values and Vision of Lad Lake including being trauma informed, culturally competent, evidenced based, community and family focused.

2. Is knowledgeable of client rights and ensures an atmosphere, which allows for the privacy,

dignity and well being of all clients in a safe, secure environment.

3. Supports, cooperates with, and implements specific procedures and programs for:

  1. Confidentiality of all data, including client, employee and operations data.
  2. Quality Assurance and compliance with all licensing and regulatory requirements.
  3. Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal and discriminatory behavior.
  4. Safety, including universal precautions and safe work practices.

4. Supports and participates in common teamwork:

  1. Cooperates and works together with all co-workers, performs job duties to the best of ability, plan and complete job duties with minimal supervisory direction, including appropriate judgment.
  2. Uses tactful, appropriate communications in sensitive and emotional situations.
  3. Follows up as appropriate with supervisor, co-workers or clients regarding reported complaints, problems and concerns.
  4. Promotes positive public relations and pleasant attitude towards clients, staff members and guests.
  5. Completes requirements for in-service training, acceptable attendance, complete time sheet records accurately, and other work duties as assigned.


ESSENTIAL DUTIES

Client Services

  1. Average caseload of 7 children, youth, and young adults with mental health diagnoses.
  2. Facilitate all areas of services for children and young adults to include, but not limited to: coordinate mental health services (therapy, medication management, peer support, wellness management, evaluations, etc.), life skill development, employment & education supports, connect to community resources.
  3. Advocate for youth/young adults across a variety of settings, including home, educational, court, and community settings, as needed.
  4. To maintain regular in-person contact with enrollees as is defined by program.
  5. The Care Coordinator works with children, families, and young adults to develop an Plan of Care, based on identified strengths and needs, including a detailed progressive reactive crisis plan.
  6. Serve as a liaison between children, families, and young adults and community providers and assure acquisition of services.
  7. To prepare, complete, and submit required documentation by due dates in Avatar to include, but not limited to: Community Progress Notes, Plan of Cares, Crisis Plans, referrals, annual Functional Screen, annual assessment, etc.
  8. To monitor and negotiate delivery of services and agreements with children, families and young adults and their teams, to include modification of Plan of Cares as needed.
  9. To establish and maintain good communication, collaboration, and cooperation with all stakeholders (i.e. Wraparound, provider agencies, BHD, etc.).
  10. To work and communicate honestly and effectively with children, young adults, families, team members, and other identified supports to develop and implement problem-solving strategies, listen to concerns, facilitate crisis services through informal, natural and formal supports in a manner reflective of both Lad Lake and Wraparound.
  11. Attends all supervision, consultation, meetings, Wraparound, Lad Lake, and CCS required trainings, in-services, events, and professional development activities as directed.
  12. Able to work flexible hours, including some evenings, depending on the needs of the children, families, young adults and program needs.

LEAD DUTIES

  1. Role model and coach on the Values and Principles of the Wraparound Process.
  2. Assist the Supervisor with providing support, feedback and problem solving with Care Coordinators, as needed.
  3. Act as a trainer in conjunction with the Supervisor to teach the Wraparound Process to new and existing staff.
  4. Instruct new staff regarding Wraparound Milwaukee policies and procedures and maintain follow through on policies with existing staff.
  5. Provide coaching for staff by attending Court hearings and/or Child & Family Team Meetings
  6. Help plan and conduct Agency team meetings and trainings.
  7. Attend Wraparound Milwaukee Supervisor/Lead Meetings.
  8. Review documentation for accuracy and strength-based language (including Progress Notes, Plans of Care, Crisis Plan/CCS Support Plans, Disenrollment Summary's)

OTHER DUTIES

  1. Contribute to the program’s resource development effort identifying new community resources available to our clients and staff and for the betterment of the program.
  2. Participate in efforts to promote Lad Lake and the CCS Program, as well as contribute to the development of the same.
  3. Participate in “additional duties as assigned” at the discretion of program management or agency administrative staff.
