CASE MANAGER 2 - ADULT SERVICES
Salary Grade: 8
Starting Salary - $22.76/hour
Shift Available - Full - Time (Non-Exempt)
Office Hours: 8:30 am to 5 pm Monday - Friday
Budgeted Hours: 7.5 hours per day / 37.5 hours per week.
Summary of Job Functions:
• Provides case management to individuals/families involved with County departments, local human service agencies, the court system, or self-referrals.
• Offers support and referrals to individuals/families who are experience a life transition and need assistance navigating the human service system.
• Assesses challenges and barriers to receiving services.
• Builds and maintains relationships with existing service providers to help individuals/families access food, receive financial assistance, resolve homelessness or near-homelessness, and prevent utility termination.
• Advocates for individuals who are involved with the court system, have mental illness, substance use disorder, and/or intellectual and development issues.
• Case management activities include client needs assessment, service coordination and client advocacy, goal setting, service follow-up, reassessment of needs, case notes, data entry, filing, and client confidentiality.
• Attends meetings, trainings, and seminars as required.
• Performs other job-related duties as instructed by the Director.
Minimum Training and Experience:
Bachelor's degree or equivalent in a human service-related field and one to two (1-2) years of experience in the social service field. Background with human service programs and agencies in Centre County, PA. Experience providing services to individuals diagnosed with serious mental illness, involved with Children & Youth, and/or individuals with physical disabilities who may also be of low income/limited resources and generational poverty. Understanding and experience working with individuals involved in the criminal justice system. Must possess a valid Pennsylvania driver's license and a willingness to travel as needed.
