Licensed Social Worker

Are you a compassionate mental health professional dedicated to guiding individuals through their recovery journeys? At RWJBarnabas Health, the Sickle Cell Social Worker provides comprehensive psychosocial support, education, advocacy, and care coordination to adult patients living with sickle cell disease. This position works closely with physicians, nurses, advanced practice providers, behavioral health professionals, and community organizations to address barriers to care and improve patients overall health and quality of life. As a Licensed Social Worker (LSW), you will perform vital clinical assessments and design individualized treatment plans tailored to help patients achieve their personal goals. As a Licensed Social Worker, a typical day may look like:

  • Conduct psychosocial assessments and develop individualized plans of care
  • Provide counseling, emotional support, crisis intervention, and assistance with adjustment to chronic illness
  • Identify and address barriers related to transportation, housing, food insecurity, insurance, employment, medication access, and financial concerns
  • Connect patients and families with hospital, community, state, and federal resources
  • Assist with insurance, disability, Family and Medical Leave Act (FMLA), medication assistance, and other benefit-related applications
  • Support patients transitioning from pediatric to adult sickle cell care
  • Collaborate with the multidisciplinary team to promote treatment adherence, continuity of care, and effective discharge planning
  • Participate in case conferences, care planning meetings, quality-improvement initiatives, and patient outreach activities
  • Provide education and advocacy to help reduce stigma and improve understanding of sickle cell disease
  • Maintain timely and accurate documentation in the electronic medical record
  • Establish relationships with community organizations and participate in sickle cell education and awareness events
  • Follow up with patients after emergency department visits, hospitalizations, missed appointments, or identified gaps in care
  • Perform other related duties as assigned

To be considered for this opportunity, you must have:

  • Masters degree in Social Work from an accredited institution required
  • Current New Jersey social work license or eligibility for licensure required
  • Previous experience in a hospital, outpatient medical practice, hematology, or chronic-disease setting preferred
  • Experience working with individuals affected by sickle cell disease is strongly preferred
  • Proficiency with electronic medical records and Microsoft Office applications preferred
  • Schedule: Full-time Monday- Friday, primarily on-site, with occasional participation in community outreach events as needed
