Behavioral Health Specialist (QMHS) – Permanent Supportive Housing in Marion, Ohio at Third Street Family Health Services

What We're Looking For

As a Behavioral Health Specialist assigned to Permanent Supportive Housing, you will provide community-based behavioral health services and care coordination to individuals participating in Third Street's Permanent Supportive Housing Program. You will help residents maintain stable housing, increase independence, access community resources, and address barriers that may affect their health and well-being.

Responsibilities include:

• Provide community-based behavioral health interventions, supportive services, and care coordination.
• Support residents in maintaining stable housing and developing independent living skills.
• Assist residents with accessing housing, healthcare, benefits, transportation, financial assistance, and other community resources.
• Complete housing-related activities, including inspections, income verifications, lease renewals, and move-in/move-out processes.
• Maintain required resident documentation and records in accordance with organizational, program, and regulatory requirements.
• Collaborate with healthcare providers, community agencies, landlords, and other partners to support resident needs.
• Assist with crisis situations and identify concerns that may affect a resident's safety, housing stability, or overall well-being.
• Use a person-centered, recovery-oriented, and strengths-based approach when working with residents.
• Participate in interdisciplinary meetings, training, and ongoing competency development.
• Maintain compliance with applicable HUD, ADAMH, Medicaid, CARF, OMHAS, HRSA, and organizational requirements.

Qualifications:

• High school diploma or GED.
• Meets Ohio Medicaid requirements for Qualified Mental Health Specialist (QMHS).
• Valid driver's license and reliable transportation.
• Ability to obtain BLS certification and complete credentialing within 90 days of hire.
