Job Description
We are seeking a passionate, motivated, and compassionate Community Health Worker (CHW) to support our Enhanced Care Management (ECM) program serving IEHP members. In this vital role, you will act as a bridge between healthcare providers and the community by helping patients navigate the healthcare system, access resources, and achieve improved health outcomes.
As a Community Health Worker, you will provide personalized support, health education, and care coordination for patients with complex medical and social needs. Your work will focus on addressing social determinants of health, empowering patients to actively participate in their care plans, and connecting individuals to community-based resources that support long-term health and stability.

Requirements:- Engage with patients and community members to assess health needs and assist in developing individualized care plans that promote wellness and independence
- Conduct outreach and engagement with IEHP members enrolled in Enhanced Care Management (ECM) programs
- Provide education on topics including nutrition, medication adherence, chronic disease management, preventive care, and healthy lifestyle practices
- Utilize motivational interviewing techniques to support behavioral change and encourage patients to actively participate in their care plans
- Assist patients in navigating healthcare systems including primary care, specialty care, behavioral health services, and community-based programs
- Identify and address social determinants of health such as housing instability, food insecurity, transportation barriers, and access to community resources
- Support patients with scheduling appointments, obtaining referrals, and connecting to local community services
- Collaborate with healthcare providers, nurses, social workers, and care managers to coordinate services and ensure comprehensive patient support
- Conduct outreach activities including community events, patient education sessions, and public engagement to increase awareness of available healthcare services
- Accurately collect and document client interactions, health status updates, and care coordination activities in the electronic health record (EHR) while maintaining patient confidentiality and HIPAA compliance
Qualifications
- High School Diploma or equivalent required
- Community Health Worker (CHW) certification preferred or willingness to obtain
- Experience working with Medi-Cal populations, care coordination, or community outreach programs preferred
- Familiarity with IEHP Enhanced Care Management (ECM) programs or managed care environments is a plus
- Strong understanding of medical terminology, healthcare systems, and community-based resources
- Experience working with individuals facing complex social determinants of health, disabilities, or chronic medical conditions
- Strong communication and interpersonal skills with the ability to build trust quickly
- Ability to work effectively with diverse populations and multidisciplinary healthcare teams
- Experience documenting services in electronic health records (EHR) or similar systems
