Case Manager Job DescriptionPosition Summary

We are seeking a compassionate, organized, and dedicated Case Manager to join our team. In this role, you will support individuals on their recovery and wellness journey by assessing needs, coordinating services, advocating for resources, and helping participants achieve meaningful goals. The Case Manager serves as a vital link between participants, families, community providers, and internal programs to ensure comprehensive, person-centered care and positive outcomes.

Key ResponsibilitiesParticipant Assessment and Care Coordination
  • Conduct comprehensive assessments to identify participant strengths, needs, barriers, and goals.
  • Develop, implement, and monitor individualized service and treatment plans.
  • Provide ongoing case management services to support participant progress and success.
  • Coordinate services across internal programs, healthcare providers, community agencies, and support systems.
  • Facilitate referrals and connect participants with appropriate treatment, recovery, and supportive services.
Advocacy and Participant Support
  • Advocate on behalf of participants to ensure access to needed services, benefits, and resources.
  • Assist participants with obtaining and maintaining essential supports, including:
    • Medicaid and healthcare benefits
    • Housing resources
    • Employment and vocational services
    • Transportation assistance
    • Community-based supports
  • Provide person-centered services that promote recovery, independence, and overall well-being.
  • Support participants in overcoming barriers and achieving individual recovery goals.
Community Collaboration
  • Build and maintain positive working relationships with community partners and service providers.
  • Collaborate with families, caregivers, treatment teams, and external agencies to ensure continuity of care.
  • Participate actively in multidisciplinary team meetings and care planning discussions.
  • Identify service gaps and recommend opportunities for program improvement.
Documentation and Compliance
  • Complete timely, accurate, and thorough assessments, case notes, service documentation, and compliance records.
  • Ensure services are delivered in accordance with funding, licensing, accreditation, and regulatory requirements.
  • Maintain participant confidentiality and adhere to HIPAA standards.
  • Support program operations and administrative case management functions as needed.
QualificationsRequired
  • Bachelor's Degree in Human Services, Social Work, Psychology, Counseling, or a related field.
  • Valid driver's license and reliable transportation.
  • Ability to work effectively in both office and community-based settings.
  • Intermediate computer proficiency, including Microsoft Office applications and electronic documentation systems.
Preferred
  • One (1) year of case management experience.
  • Two (2) years of experience supporting individuals in behavioral health, mental health, substance use disorder treatment, or addiction recovery services.

  • Supports participant access to services, benefits, and community resources.
  • Demonstrates knowledge of local support systems and recovery-oriented services.
Documentation and Compliance
  • Maintains accurate, timely, and compliant records.
  • Understands and follows regulatory, funding, and accreditation standards.
Cultural Competence
  • Provides respectful, inclusive, and person-centered services to individuals from diverse backgrounds.
  • Recognizes and values each participant's unique experiences, choices, and goals.
Work Environment and Physical Requirements

This position requires a combination of office-based and community-based work and may involve travel throughout the service area.

Physical Requirements
  • Prolonged sitting, standing, and driving.
  • Frequent bending, reaching, stooping, and climbing stairs.
  • Ability to lift and carry up to 50 pounds.
  • Ability to travel independently between participant appointments and agency locations.

  • Copier, fax machine, calculator, and other standard office equipment
