Case ManagerHelp Individuals Navigate Resources, Achieve Goals, and Build Brighter Futures

Position Summary

The Case Manager is responsible for assessing participant needs, coordinating and implementing individualized service plans, and advocating to ensure access to necessary services and resources. Through a recovery-oriented and person-centered approach, the Case Manager helps participants achieve their goals, improve their quality of life, and successfully navigate community systems and supports.

Working collaboratively with participants, families, interdisciplinary teams, and community providers, the Case Manager plays a vital role in promoting positive outcomes and long-term success.

Essential ResponsibilitiesParticipant Assessment and Service Planning
  • Assess participant strengths, needs, barriers, and goals.
  • Identify appropriate services, resources, and supports based on individual needs.
  • Develop, implement, and monitor individualized service and treatment plans.
  • Support participants in achieving recovery, wellness, and self-sufficiency goals.
  • Conduct screenings and referrals to appropriate programs and providers.
Case Management and Care Coordination
  • Coordinate services among healthcare providers, behavioral health professionals, community agencies, and support systems.
  • Facilitate access to treatment, housing, employment services, transportation, and other community resources.
  • Assist participants with obtaining benefits and entitlements, including Medicaid and other public assistance programs.
  • Monitor participant progress and adjust service plans as needed.
  • Collaborate with participants, families, and service providers to ensure continuity of care.
Advocacy and Resource Navigation
  • Advocate on behalf of participants to remove barriers and improve access to services.
  • Help participants navigate complex healthcare, behavioral health, and social service systems.
  • Connect individuals with community resources that support long-term recovery and stability.
  • Promote participant self-advocacy and empowerment.
Additional Responsibilities
  • Participate in multidisciplinary team meetings and care planning discussions.
  • Build and maintain positive relationships with community agencies and referral partners.
  • Identify service gaps and provide recommendations for program improvements.
  • Complete all required assessments, case notes, documentation, and compliance records.
  • Coordinate services across internal programs and external agencies.
  • Ensure compliance with all funding, licensing, accreditation, and organizational requirements.
  • Support program operations and administrative case management activities.
  • Attend required trainings, meetings, and professional development opportunities.
QualificationsRequired
  • Bachelor's Degree in Human Services or a related field.
  • Valid driver's license with reliable transportation.
  • Intermediate computer skills, including proficiency with Microsoft Office applications.
  • Ability to work effectively in both office and community-based settings.
Preferred
  • One (1) year of case management experience.
  • Two (2) years of experience supporting individuals in:
    • Behavioral health services
    • Addiction recovery services
    • Community-based human services programs
    • Social services or related fields

Ability to maintain accurate records and documentation while meeting regulatory and funding requirements.

Cultural Competency and Person-Centered Practice

Commitment to providing services that respect the diverse backgrounds, experiences, and values of all participants.

Working Conditions

This position combines office-based and community-based work and may require:

  • Travel to participant homes, community locations, and service provider offices.
  • Collaboration with external agencies and community partners.
  • Frequent use of computers, telephones, and other office equipment.
  • Flexibility to meet participant and program needs.

Work environments may include modern office settings, community agencies, healthcare facilities, and residential locations.

Physical Requirements

The employee must be able to:

  • Sit, stand, and walk for extended periods.
  • Lift and carry up to 50 pounds when necessary.
  • Reach, bend, stoop, twist, and climb stairs.
  • Drive a motor vehicle for community-based services and meetings.
  • Use standard office equipment including computers, telephones, copiers, and scanners.
  • Work in multiple office and community locations.
