Home Care Coordinator

POSITION SUMMARY: The Homecare Coordinator is a core member of the PACE Interdisciplinary Team responsible for coordinating, overseeing, and monitoring homecare services delivered by internal staff and contracted vendor agencies. This role ensures that authorized homecare services are delivered in accordance with participant care plans, regulatory expectations, and organizational policies.

The Homecare Coordinator actively participates in IDT care planning, assessment, and reassessment processes; monitors service delivery and aide performance; identifies care gaps or changes in participant status; and facilitates timely care plan modifications.

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

Essential Responsibilities – IDT Participation & Care Planning

  • Serves as an active and consistent member of the PACE Interdisciplinary Team.
  • Participates ininitial, annual, semi‑annual, unscheduled, and Significant Change in Condition (SDR) assessments.
  • Collaborates with nursing, therapy, social work, and providers to develop and update care plans.
  • Identifieschanges in participant status and initiates IDTreassessmentdiscussions.

Homecare Service Coordination, Care & Oversight, Center Support

  • Coordinates authorized homecare services in alignment with approved care plans.
  • Overseesinternal and vendor‑provided aide services.
  • Monitors timeliness, consistency, and adherence tocareplantasks.
  • Serves as the primary point of contact for scheduling and coverage issues.
  • Provides direct careinhome to support participant needs when necessary.
  • Serves asassistfor in-center activities or participant care if delegated by Center Leadership.

Documentation, Authorization & Compliance

  • Ensuresaccurateandtimelycompletion of aide documentation.
  • Reviews records for consistency with authorized services.
  • Communicatesmissed care or refusals to the IDT.
  • Creates and updates homecare authorizations as needed.

Aide Onboarding, Education & Support

  • Coordinates onboarding and orientation for internal aides andresourceforvendor.
  • Provides ongoing coaching and support for complex cases.
  • Identifieseducation gaps andfacilitatestraining.

Vendor Agency Coordination

  • Serves asliaisonbetween the PACE program and contracted agencies.
  • Communicatescare plans, schedules, and expectations.
  • Monitors vendor performance and escalates concerns.

Quality, Incident & Grievance Management

  • Monitors andreportsincidents, grievances, and aide change requests.
  • Participatesin audits and quality improvement activities.
  • Supports participant satisfaction follow‑up.

EXPERIENCE AND EDUCATION:

  • Experience in mentorship, lead roles, or team coordination(strongly preferred)
  • CNA, HHA, or equivalent experiencerequired, LPN or supervision experience strongly preferredor other relatedexperience working within a care team model.
  • 1–3 years homecare or PACE experience preferred.
  • Experience providing personal care to frail elderly adults (strongly preferred)

PRE-EMPLOYMENT REQUIREMENTS:  

  • Must have reliable transportation, a valid driver's license, and the minimum staterequiredliability auto insurance.   
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.   
  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
