About the National Kidney Foundation of Michigan:
The National Kidney Foundation of Michigan (NKFM) is a nonprofit organization dedicated to improving the health and well-being of individuals affected by kidney disease. NKFM focuses on kidney health education, prevention, and research while offering support to kidney patients and their families. We are committed to increasing public awareness, promoting healthy living, and advancing medical research related to kidney disease.

Position Summary
HBP Control leaders are employed by the National Kidney Foundation of Michigan (NKFM). The NKFM is a licensed provider of the Health Coaches for Hypertension Control programs, referred to as HBP Control in Michigan. HBP Control leaders are paid a set 3.5 to 4 hours for each session that includes preparation and administrative tasks each week. Leaders facilitate groups of participants through 90-minute sessions held once per week for 8-weeks using a standardized curriculum. Workshops are held both in-person and virtually. Leaders provide support to participants with strategies to manage their blood pressure, overall health, and help participants set practical, achievable goals each week. Training in the curriculum is provided. Leaders are individuals who are successfully managing their hypertension or are a caregiver or support person for someone with hypertension.

Qualifications
• Ability to communicate effectively with individuals and in front of a small group;
• Active listening skills; enthusiasm and a positive attitude;
• Ability to be non-judgmental;
• Ability to work with a diverse group of people of varying ages, ethnicities, experiences and geographic locations;
• Ability to implement a standardized curriculum effectively;
• Proficient in MS Word, MS Excel, email and online virtual communication platforms;
• Access to a computer, tablet, or smart phone with reliable internet for communication with NKFM staff and with workshop participants using an NKFM provided email;
• Must be willing to deliver programs in-person in the geographic communities of focus;
• Physical requirements - physical effort equal to transferring materials between a vehicle and a cart up to 30 pounds. Regularly required to sit or stand, bend and reach;
• Must be legally eligible to work in the United States.

Key Responsibilities
• Facilitate at least 1 workshop assigned by the NKFM in-person or virtually within 12 months of becoming a leader and annually thereafter;
• Attend and complete training to become certified as a leader;
• Create an environment that is motivating, supportive, non‐competitive, and non‐judgmental;
• Engage group dynamics and group discussion among participants;
• Support participants by encouraging progress; setting goals; and working with the group to address challenges;
• Prepare for each session by reviewing content;
• Deliver the PATH Program as it was designed. Leaders can add their personal strengths and provide culturally relevant examples of food, types of physical activity, etc. but should not alter the curriculum;
• Maintain high standards and expectations of self and participants including attendance, starting on time, self‐monitoring, and problem solving;
• Record and submit attendance to NKFM within 48 hours;
• Attend leader support training sessions to maintain and enhance skills.
• Collaborate with and respond to NKFM’s guidance to develop and enhance skills.
