Position Overview:
As RN, you will play a pivotal role in the delivery of exceptional healthcare services. You will col-laborate with a multidisciplinary team of healthcare professionals to develop and implement in-dividualized care plans, ensuring the highest standards of nursing practice are adhered to. Utiliz-ing your expertise, dedication, and compassion, you will make a positive impact on the lives of our patients and their families.

Responsibilities:
 A Registered Nurse will conduct thorough assessments of the patient and their family, using physical exams, medical history reviews, and OASIS data elements to create a care plan.
 Conduct in-home visits, provide skilled nursing care, evaluate care plan effectiveness, make necessary adjustments, and perform nursing skills and procedures within allowed scope of practice.
 Provide compassionate and patient-centered care, addressing patients' physical, emotional, and psychosocial needs.
 Educate patients and their families on disease prevention, treatment options, and wellness promotion.
 Perform in-home visits, administer physician-prescribed treatments, regularly communicate patient care and progress to physician and Clinical Manager, monitor and report any unusual reactions or changes in patient condition, and document all observations and treatments ac-cording to Agency protocols.
 Identify discharge planning needs as part of the plan of care development and implement pri-or to discharge of the patient.
 Maintain accurate and up-to-date electronic health records, following all regulatory require-ments and ensure confidentiality.
 Participates in case conferences, team meetings, staff meetings, and Performance Improve-ment activities as assigned.
 Follow infection control protocols for a safe environment. Comply with Agency policies and HIPAA Privacy rule.
Requirements:
 Valid and unrestricted professional license. Must be licensed in the State of Florida as a Regis-tered Nurse or Licensed Practical Nurse.
 RN - Bachelor's or Associate's Degree in Nursing from an accredited institution.
 One (1) to two (2) years of nursing experience required. Home health care experience pre-ferred.
 Current BLS certification.
 Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
 Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
 Ability to work independently and adapt to varying schedules.
 In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamina-tion etc.) and willingness to always follow the Kinnser EMR system (experience preferred).
