Schedule: Full-Time | Day Shift | Monday-Thursday 8:00 am - 5:00 pm Friday 8:00 am - 12:00 pm

What You’ll Do
The Registered Nurse Clinician delivers comprehensive nursing care to CAN Community Health, Inc. patients using evidence-based practice, clinical judgment, and collaboration with other members of the patient care team. The RN Clinician works under the supervision of a provider and is responsible for assessment, planning, implementation, and evaluation of care. Additionally, the RN Clinician empowers wellness in the patients we are privileged to serve by fostering a collaborative, supportive environment that encourages patient engagement, autonomy, and informed participation in their health journey.
- Perform comprehensive and focused patient assessments, including history-taking, physical examination, and interpretation of clinical data
- Develop, implement, and evaluate individualized care plans in collaboration with patients, loved ones, and the provider
- Administer medications, treatments, and procedures according to established protocols and scope of practice
- Monitor and document patient responses, promptly recognizing and responding to changes in condition
- Provide direct care for acute conditions including STIs, chronic conditions including HIV, and preventative services like PrEP and annual care visits.
- Ensure infection prevention and control standards are maintained
- Coordinate with physicians, physician assistants, nurse practitioners, medical assistants, and other team members to ensure continuity and quality of care
- Communicate effectively with patients, loved ones, and providers regarding care plans and progress
- Provide health promotion, risk reduction, and disease prevention counseling
- Participate in quality improvement initiatives

Education/Professional:
- Current, unrestricted license as a registered nurse (RN) in the relevant jurisdiction
- Bachelor of Science in Nursing preferred; associate degree in nursing accepted with relevant experience
- Minimum 1-2 years of recent clinical experience in an ambulatory care setting, primary care or relevant specialty
- Certification in Basic Life Support required
- Must be able to operate a motor vehicle and have valid insurance and driver’s license.
