Care Coordinator

The care coordinator will work in the office assisting the field staff in their collaboration with physicians and required services. This person is responsible for monitoring adherence to Federal and State regulations as well as ensuring therapy staff has the resources they need to care for the patient. Occasional day traige telephone call and coverage for field nurses will be needed. A degree in nursing or physical therapy is required. The ideal candidate is a Registered Nurse or Physical Therapist with strong computer skills. 1-2 yrs acute care experience and 1 yr previous home care experience preferred; the ideal candidate will have ICD-9 coding experience.
