Qualifications:

• Graduate of an accredited School of Nursing
• Current Indiana RN license
• Obtain and maintain current CPR certification and other skills required by agency
• Minimum one-year of prior Acute Care or Long Term Care Nursing experience

Responsibilities:

The Nurse Case Manager will evaluate, educate, and administer skilled nursing care to meet the physical and psychological needs of patients in their homes. This role will collaborate with patients, families, physicians, and multi-disciplinary team members to establish and follow an individualized plan of care to promote best patient outcomes. Overseeing a patient’s care from agency admission to discharge allows for both continuity of care and autonomy in nursing management.
