Acute Pharmacy Resident, PGY1 at Holzer Health System – Gallipolis, Ohio

Works under the supervision of the Director/Manager/Clinical Pharmacist of the Acute Care Inpatient Pharmacy. Evaluates medication therapy, makes medication therapy recommendations, performs medication reconciliation, follows high risk patients and collaborates in medication provisions and education. Fills, compounds, and dispenses pharmaceuticals as prescribed by the provider in accord with all legal requirements and to provide pharmaceutical information to patients and allied health professionals. Develops necessary documentation for ASHP accreditation of residency program.

Education, Work Experience and Licensure:

• Must be a graduate of an accredited college of pharmacy.
