Home Infusion Part Time Clinical Staff Pharmacist at Capstone Health Ohio LLC – Dayton, Ohio

Vital Care of Dayton is looking for a pharmacist with both dispensing and clinical experience to join our home infusion pharmacy team. This is a 20 hour per week part-time position.

Job Overview

The clinical staff pharmacist will provide clinical and operational services to home infusion and specialty pharmacy patients. The job includes overseeing sterile IV compounding, dispensing medications and appropriate supplies, evaluating prescriber orders, and patient management. The pharmacist will supervise trained and competent sterile compounding technicians and ensure compliance with USP 797 and accreditation guidelines. Additionally, the pharmacist will review clinical information, make recommendations, review lab results and perform assessments with home infusion and specialty patients. The pharmacist may participate in our quality assurance and performance improvement program.

This position reports to the Pharmacy Manager.

Qualifications

Required:

• Licensed pharmacist in good standing with the Ohio Board of Pharmacy.
• Good interactive skills with coworkers, referral sources, physicians, and our clients

Preferred:

• Three (3) years clinical experience 
• Minimum one (1) year experience with infusion therapy and sterile compounding
• Sterile compounding training
• Experience with management of pharmacy support personnel
• Multistate licensure or willingness to reciprocate to other states as needed per Capstone Leadership

Professional Responsibilities

• Oversee medication preparation and dispensing
• Patient education, clinical management, assessments, drug utilization review, and improve patient quality of care
• Coordinate with other health care providers to ensure optimal patient care
