Job Description
JOB SUMMARY:
Responsible for overall daily operations of facility. The LPN will work with a team of health care staff responsible for implementation of each member’s individualized care along with providing oversight of program activities, staffing, supervising, and training staff and ensuring adherence to compliance and regulation standards.
GENERAL REQUIREMENTS:
- Performs all job responsibilities in alignment with the mission and vision of the organization.
- Performs other duties as required and completes all job functions as per departmental policies and procedures.
- Maintains current knowledge in present areas of responsibility (i.e., self-education, attends ongoing educational programs).
- Attend staff meetings and complete mandatory in-services and requirements and competency evaluations on time.
- Wear protective clothing and equipment as appropriate.
GENERAL SKILLS:
- Ability to communicate in English, both verbally and in writing.
- Additional languages are preferred.
- Strong written and verbal skills.
- Basic Computer Skills
WORKING CONDITIONS:
- General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels.
- May be exposed to high noise levels and bright lights.
- May be exposed to limited hazardous substances or body fluids, or infectious organisms.
- May be required to change from one task to another or different nature without loss of efficiency or composure.
- Periods of high stress and fluctuating workloads may occur.
- May be scheduled as needed including overtime.
PHYSICAL REQUIREMENTS & DEMANDS:
- Have near normal hearing: Hear alarms/telephone/normal speaking voice.
- Have near normal vision: Clarity of vision (both near and far), ability to distinguish colors.
- Have good manual dexterity.
- Have good eye-hand foot coordination.
- Ability to perform repetitive tasks/motion.
- Continuously within shift (67-100%): Standing, Walking.
- Frequently within shift (34-66%): Bending/Stooping, Pushing/Pulling, Lift/carry up to 20lbs, Lift/carry greater than 20 lbs. with assistance.
- Occasionally within shift (1-33%): Sitting, Climbing, Twist at waist, Lift/Carry greater than 50 lbs. with assistance, Reaching above shoulder.

JOB SPECIFIC COMPETENCIES:
- Observes patients, charts, and reports changes in client’s condition.
- Administers prescribed medication and notes on client’s medication administration record.
- Supervises caregivers and dietary aide.
- Assists with day-to-day activities for increased quality of life to the residents.
- Prepares weekly billing report.
- Assists Administrator with budgeting and recording expenditures.
- Prepare monthly reports to case management and care coordination agencies.
- Assists RN with admission process.
- Completes monthly summaries on clients in a timely manner.
- Conducts monthly disaster drills.
- Assists Executive Director with budgeting and recording expenditures.
- Ensures the provision of staff department orientation and continuous training/education and maintains proper personnel records.
- Conducts monthly disaster drills biannually.
- Ensures proper client/staff ratio.
- Ensures the successful transportation of clients to and from the facility.
- Assists Executive Director in ensuring adherence to state rules and regulations.
- Maintains client charts and keeping client records current.
- Completes incident reports when necessary and fully cooperate with any investigation and corrective plan.
- Documents client’s status and records
- LPN should complete a 30-day supervisory visit on each client. If client is not present on the date the visit is due, the visit should be completed the next day the client attends. Information included: member transport, ambulatory status, where he/she resides, how many days they attend per week, and the statement of satisfaction (direct quote from the client), and any changes in the client’s condition.
- If the client receives any medication, skilled nursing (tube feedings) this should be noted.
- At each LPN visit, the care plan and MAR should be signed on the same date that visit was conducted by the LPN.
- The Activities of Daily Living (ADLs) sheets should be reviewed by the LPN and signed on the last day of the month that the client attended.
- ADL sheets must be completed accurately and in their entirety.
- Completing incident reports when necessary and fully cooperating with any investigation and implementing findings
PROFESSIONAL REQUIREMENTS:
- Follows Code of Conduct policy.
- Adheres to dress code; appearance is neat and clean.
- Completes annual educational requirements.
- Maintains regulatory requirements.
- Maintains patient confidentiality at all times.
- Reports to work on time and as scheduled; completes work within designated time.
- Wears identification when on duty; use computerized time clock system correctly.
- Completes in-services and returns in a timely fashion.
- Attends annual review and/or skills fair and department in-services, as scheduled.
- Attempts to end conversations and other interactions in a positive manner; leaves others with a good impression of the Hospital Authority of Miller County and its employees.
- Complies with all organizational policies regarding ethical business practices.
- Communicates the mission statement of the organization.

Requirements:EDUCATION, CREDENTIALS & EXPERIENCE REQUIREMENTS:
- Current LPN licensure in State of Georgia
- Active American Heart Association BLS certification within 30 days of hire.
