RN Care Coordinator – Field Based in Dothan, Alabama at FirstLight Home Care of Dothan

FirstLight Home Care of Dothan is hiring a field-based RN Care Coordinator to meet with clients in their homes, build relationships with families, and help guide care decisions throughout the Wiregrass area.

This is not a traditional nursing role. We are looking for someone who genuinely enjoys connecting with people, working independently, and being out in the community.

What You’ll Do:
- Conduct in-home assessments for new and existing clients
- Build relationships with clients and families
- Help families feel confident moving forward with care
- Complete required VA RN assessments (training provided)
- Visit current clients and check in on care needs
- Work closely with scheduling and leadership teams
- Manage leads and follow-ups using our CRM system

Compensation:
$50,000 base salary + Performance Bonuses

Performance bonuses for:
- New clients brought on
- Increased client hours
- VA assessments completed on time

Top performers can realistically earn $60K–$65K+ annually.
