As a Home Health Physical Therapist, you'll enjoy the autonomy of treating patients one-on-one in their homes while working alongside an experienced interdisciplinary team dedicated to delivering exceptional care.

Position Details & Compensation

IMPORTANT: This is a 1099 Independent Contractor position.

  • PT Evaluation: $125 per visit
  • PT Follow-Up: $80 per visit
  • Start of Care (SOC): $160 per visit
  • Mileage reimbursement: $0.50 per mile
  • PRN, Part-Time & Full-Time opportunities available
  • Flexible scheduling
  • Active professional malpractice insurance is required

As a 1099 Independent Contractor, the Physical Therapist is responsible for maintaining active professional malpractice insurance, a valid professional license, required certifications, and personal automobile insurance throughout their engagement with Vision Healthcare, LLC.

Position Summary

The Physical Therapist is responsible for providing skilled physical therapy services to patients in their homes. Responsibilities include performing comprehensive evaluations, developing individualized treatment plans, improving functional mobility, reducing pain, restoring independence, and collaborating with physicians and the interdisciplinary care team to achieve the best possible patient outcomes.

Responsibilities
  • Perform comprehensive physical therapy evaluations.
  • Develop individualized treatment plans based on physician orders and patient goals.
  • Provide therapeutic exercises, gait training, balance training, strengthening, transfer training, and neuromuscular re-education.
  • Evaluate home safety and recommend environmental modifications to reduce fall risk.
  • Assess the need for durable medical equipment (DME) and assistive devices.
  • Educate patients and caregivers on home exercise programs, safety, disease management, and injury prevention.
  • Monitor patient progress and adjust treatment plans as appropriate.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and other healthcare professionals.
  • Complete accurate and timely electronic documentation.
  • Participate in interdisciplinary case conferences and quality improvement initiatives.
  • Maintain compliance with physician orders, agency policies, and applicable Virginia and payer requirements.
Qualifications & Requirements

Required:

  • Graduate of an accredited Physical Therapy program.
  • Current, unrestricted Virginia Physical Therapist license.
  • Current CPR/BLS certification.
  • Active professional malpractice insurance.
  • Valid driver's license.
  • Reliable transportation.
  • Current automobile insurance.
  • Ability to travel throughout assigned service areas.
  • Excellent communication and interpersonal skills.
  • Ability to work independently in a home care setting.
  • Basic computer skills and EMR experience.

Preferred:

  • Home Health experience.
  • Medicare and Medicaid experience.
  • OASIS documentation experience.
  • Experience using Alora or similar electronic medical record systems.
