Job Overview
The Social Worker (MSW) manages the overall care coordination of pediatric patients and their families from admission through discharge. Operating within the Nursing Administration department at Children’s Specialized Hospital, this professional acts as a primary liaison between families, the multidisciplinary clinical team, physicians, and external agencies. The role focuses on guiding families through the care continuum, monitoring treatment compliance, and building robust discharge plans. Additionally, this position maintains crucial contact with third-party payers to secure ongoing funding and maximize resource utilization for the child's long-term care.
Qualifications
Required:
- Bachelor’s Degree in Social Work, Nursing, or a related Allied Health Care field.
- Minimum of three (3) years of clinical experience in a pediatric setting (OR a minimum of one (1) year of pediatric experience if holding a Master's Degree/MSW).
- Active American Heart Association (AHA) BLS Provider Certification.
- Compliance with all professional licensing and accrediting standards.
- Strong baseline of cognitive analytical, problem-solving, and organizational skills to manage multiple tasks simultaneously.
Preferred:
- Master of Social Work (MSW) or equivalent Master's Degree in a related field.
- Strong background navigating managed care systems, health care resource utilization, and third-party payer systems.
Scheduling Requirements
- Full-Time (FT) - Day
- Exempt status. Occasional travel to referring/receiving hospitals or external community agencies may be required.
Essential Functions
- Care Coordination: Engages patients and families in professional helping relationships; coordinates multidisciplinary team functions to achieve unified treatment and discharge goals.
- Clinical Liaison & Advocacy: Transmits complex medical diagnoses and prognosis information accurately to external agencies; advocates for patients while fostering the family's independence.
- Financial Oversight: Continually interfaces with Pre-Admissions, Finance, Health Information Management (HIMS), and Utilization Review to communicate financial/social barriers and secure ongoing funding from third-party payers.
- Relationship Management: Enhances the facility's mission by developing strong partnerships with community groups, government agencies, service providers, and hospital referral sources.
- Risk Assessment & Safety: Demonstrates a high degree of customer sensitivity; identifies families at high risk for child neglect or abuse and initiates mandatory regulatory referrals.
- Documentation & Compliance: Completes thorough, legible, and timely medical record documentation; prepares and submits necessary clinical files to external agencies.
- Department Support: Participates in strategic hospital projects and initiatives that align with department performance goals and the overarching mission of the healthcare system.
