Director of Population Health, Value-Base Care & Clinical Integration

Crouse Health’s Clinically Integrated Network team is hiring a Director of Population Health, Value-Base Care & Clinical Integration.
Pay Range: $170,000.00 - $200,000.00 / annual
Schedule: Full-Time, Monday-Friday, 8:00am-4:30pm
Position Responsibilities:
- Develops internal systems utilizing appropriate resources to support the operating needs of the assigned department(s) of responsibility including, but limited to: finance, planning, marketing, advertising, information systems, human resources and other support systems. Coordinates financial and budgetary acquisitions, allocations and utilization of resources needed for quality, cost-effective, program(s)/service(s).
- Provides strategic guidance to leadership staff within all areas of responsibility.
- Develops, plans, and evaluates short, mid and long-term goal setting and goal achievement through facilitation and guidance consistent with the strategic plan.
- Maintains current industry knowledge serving as a subject matter expert; ensures compliance with all federal/state/local laws, regulatory/accrediting guidelines and professional standards within all areas of responsibility.
- Attends and actively participates in hospital and performance improvement councils and committees as requested.
- Proactively communicates with staff and keeps them informed on a regular basis or as needed; updates staff regarding policy changes in specialized areas.
- Develops and utilizes department policy statements, scope of service, quality indicators and updates; revises as necessary.
- Performs staff management functions related to employee selection, on-boarding/orientation, coaching/development, staffing, work assignments, performance management and goal alignment as guided by hospital policy and procedures and collective bargaining unit agreement (when applicable).
- Participates, collaborates and actively engages in meetings to achieve goals and mutually beneficial outcomes.
- Conducts regular staff meetings to disseminate hospital and/or department/division information and allows for discussion of staff concerns needing clarification or resolution.
- Stays abreast of industry and market trends, communicates trends to the boards and recommends modications in direction, as needed.
- Develops and maintains eective, comprehensive utilization management and quality management systems with components which facilitate credentialing, utilization review, costs data, management, member performance feedback and communications and other administrative functions.
- Assists the boards and their committees in identifying, communications and promoting methods to improve quality of health care provided by its members.
- Develops and maintains an eective user grievance system which assures timely response and resolution.
- Fosters and encourages good public relations with all current and potential customers. Initiates, develops and maintains cooperative relationships with local business community.
- Recommends policies for board approvals. Communicates and insures compliance with policies and procedures.
- Assures compliance with Federal and state regulations.
- Identify key needs across Crouse Health’s member populations and develop scalable, data-driven programs to improve outcomes and reduce the total cost of care for population health.
- Promote collaboration and alignment across physicians, practices and health systems partners.
- Design evidence-based protocols to streamline chronic disease management.
- Translate utilization, cost, and quality data into actionable insights for physicians.
- Support payor partnerships and serve as the primary point of contact.
- Facilitate process improvement activities in support of clinical integration initiatives.
- Other duties as assigned by Senior Leadership.
Position Requirements:
- Bachelor’s degree in a related field required.
- Master’s degree (MS, MPH, MBA) or other licensed clinical degree (LPN, RN, NP) preferred.
- Minimum five years’ experience in a related field with prior experience in population health, value-based care performance, clinical integration, utilization and care management, physician engagement and working directly with payer partners.
- Must have experience managing projects in health care quality improvement and management, performance improvement, and/or practice transformation.
- Proven business acumen and analytical skills including an understanding of clinical data management and analysis.
- Excellent written and verbal communication skills and strong facilitative interpersonal skills.
- Excellent organizational and problem-solving skills.
- Current knowledge of governing regulations, third party payer utilization, quality mandates, reimbursement requirement and standards associated with Care Management and Utilization Management.
- Demonstrated ability to interact with physicians, patients, healthcare providers and team members.
