Livingston Memorial is looking for a Full Time, 40 hours a week, Wound Care Certified Nurse (WCC) to support our Home Health Team.

Position Summary:
This position requires a skilled clinical practitioner who exhibits exceptional wound management skills and is highly proficient at consultation and education. The WCC assesses and monitors wounds and works collaboratively with other members of the patient care team, to determine if additional treatments or changes in care are required. The WCC Educates patients and caretakers on wound care, infection and injury prevention, as well as pressure ulcer care. This Position provides consultation to clinical staff related to wound care and gives recommendations for specific wound care and wound management for Home Health and Hospice patients. This position provides Educational classes to field staff on wound care and advanced wound care.
What You'll Do
- Mentors staff in the management of wounds. Gives advice on suitable wound care for particular wounds.
- Provides educational classes for the staff as needed in coordination with the Director of Education and in collaboration with the CWON, if available.
- Provides education and instruction to patients and their families.
- Participates and instructs in specialty related in-services for the organization, and on occasion, external professional organizations/associations as appropriate/required.
- Participates in QI process related to specialty services.
- Demonstrates knowledge of OASIS documents, performs OASIS visits as needed on appropriate patients (ostomy or difficult wounds).
- Interacts with insurance coordinator and DME provider as appropriate.
- Proactively ensures patients have correct supplies.
- Attends and participates in Case Conferences and Staff meetings.
- Communicates regularly with Supervisor about department issues and suggests improvements.
- Demonstrates flexible and efficient time management and ability to prioritize workload.
- Monitors case communication follow up notes, qliqs, messages, and e-mails and takes appropriate action in a timely fashion.
- Demonstrates assertiveness, leadership and good interpersonal skills with physicians and clinicians. Maintains good internal and external customer satisfaction.
- Maintains a positive and respectful professional attitude.
- Acts as a positive role model within and outside the agency
- Performs other related duties as assigned
What We're Looking For
- Current Wound Care Certified (WCC) Certificate or Wound Care & Ostomy Certified (WCON) from an accredited institution
- Current RN license for the state of California.
- Current BLS Certificate
- Two years experience in home healthcare or acute care in this specialty.
