Summary of Job Function This position is within the Comprehensive Hemophilia Treatment Center, treating both adult and pediatric populations. The Adult and Pediatric or Family Acute Care Advanced Practice Nurse for Medicine (APN) is an independent health care provider who coordinates inpatient and outpatient care through collaboration with the division chief or his/her designee They will organize and administer individualized age specific professional care to patients undergoing diagnostic, treatment and procedures in the patient care setting with Hemophilia and other coagulation disorders Provides advanced practice nursing care as directed by the medical staff and in accordance with the objectives, policies, and procedures of the hospital and the State Nurse Practice Act, Maintains the standards of advanced practice nursing care as established and participates in patient teaching and discharge, Participates in unit Quality Improvement and educational activities, Uses an understanding of the principles of growth and development to assess each patient's age specific needs and provide age specific treatment Job Qualifications-
Currently registered professional nurse in the State of New Jersey
Family Nurse Practitioner Certification and License
Current BLS Heart Saver/Health Care Provider Certification
Experience with Hemophilia or Coagulation Disorders preferred
Experience with both pediatric and adult populations preferred

Performs patient care according to established policies and procedures; adapts techniques to unique needs of individual patients
Seeks assistance from doctors as necessary in solving clinical and management problems
Demonstrates effectiveness in coordinating aspects of patient care involving other departments and assists in scheduling various appointments
Obtains an appropriate History and Physical of assigned patient on admission; utilizes all available sources of information (Patient, Medical Record, Other Professionals); gathers and incorporates all information necessary to properly assess the patient
Reviews lab work, communicates abnormal values to physicians, orders redraws as needed
Transmits pertinent patient information to appropriate physician, discharge planner, etc
Consults and work collaboratively with Hemophilia Treatment Center team including RN, Research and Financial coordinators, physicians, RNs, Social Workers, Data Analysts, Program and Nurse managers, Coagulation lab Supervisor and Technician, and other members of the patient care team
Develops a teaching & discharge plan for individual patients
Communicates assessment data and findings with other members of the patient care team
Demonstrates competence in performing assigned advanced nursing procedures and education for the comfort and well-being of patients
Serves as a patient advocate
Evaluates effectiveness of interventions and revises plan of care accordingly
