About This Position
To provide a dual role in supporting Population Health initiatives and delivering direct clinical nursing care. This position shares responsibilities between patient care duties on the floor and Population Health tasks, including employee health, clinical education, and infection control surveillance activities.
Key Responsibilities:

Deliver safe, evidence-based patient care within scope of nursing practice.

Document clinical activities in compliance with regulatory standards and facility protocols.

Respond to patient needs and emergencies while collaborating with interdisciplinary team members.

Participate in patient education, discharge planning, and patient/family communication as needed.

Assist with onboarding and competency assessment of clinical staff.

Deliver and track annual education, mandatory training, and in-services.

Support employee health documentation (vaccinations, TB, Hepatitis B etc.).

Assist in contact tracing, post-exposure follow-up, and monitoring health compliance.

Perform infection control surveillance, including hand hygiene audits and observational rounds.

Participate in infection control committees as needed.

Adhere to confidentiality, HIPAA, and safe work practices.

- Adheres to established safety protocols, infection prevention standards, and hand hygiene practices to reduce the risk of harm to patients, residents, visitors, and staff.
- Participates in proactive risk identification and timely reporting of adverse events or near misses.
- Supports and educates staff in maintaining a culture of safety across nursing, population health, and employee wellness activities.
- Follows isolation precautions, immunization protocols, and surveillance workflows in compliance with regulatory standards.
- Promotes safe patient care practices during clinical assignments on the floor, including medication administration, documentation, and interdisciplinary communication.
Qualifications:
Education: Current RN or LPN license in the State of Wisconsin.
Skill, Knowledge and Abilities:
- BLS certification required.
- Certified in Infection Control (CIC) preferred, not required.
- Strong clinical assessment and documentation skills.
- Excellent interpersonal and communication skills.
- Proficiency in Microsoft Word, Excel, PowerPoint.
- Experience in staff education, employee health, or infection prevention preferred.
- Ability to function independently and manage dual roles efficiently.
Experience: Familiarity with employee health, education, or infection control procedures is a plus.
