About This Position
We are seeking a highly skilled and dedicated Medical Provider (Physician) to join our healthcare team. This role offers an opportunity to deliver comprehensive patient care across various specialties, utilizing advanced medical technologies and EMR/EHR systems. All duties and responsibilities of the physician’s position are to be performed in compliance with Damian’s policies and procedures, and NYS and federal laws and regulations governing the scope of medical practice.
Essential Responsibilities:
Performs history and physical examinations in accordance with current standards of professional practice, regulatory requirements and organizational policies.
Assesses patients’ condition and age specific medical needs; sets goals; prescribes appropriate clinical interventions to meet those goals that reflect the physical, psychological, social, cultural and rehabilitative needs of the patient.
Provides complete and appropriate documentation regarding clinical observations, diagnoses, treatment objectives, and patient’s response to treatment regimens. Will utilize an outcome-oriented approach to assess patient’s response to prescribed regimens.
Identifies patient’s educational needs, and utilizes appropriate resources to meet these needs.
Collaborates effectively with other health care providers in planning interventions that will best meet the patient needs in a timely manner and promote the continuum of care.
Prescribes appropriate laboratory/diagnostic procedures related to the patient’s health status and facilitates appropriate treatment interventions predicated on results of these procedures.
Performs direct patient care and procedures such as, venipuncture and on-site laboratory diagnostic tests (waived testing), Pap smears, in accordance with DFCC policies and procedures, acceptable standards of practice and in accordance with approved delineation of privileges.
When there is no RN on duty, demonstrates the ability to direct nursing support staff in setting priorities, organizing workflow and providing quality care on a timely basis.
Maintains open communication with all members of the health care team to promote a collaborative and collegial relationship within and between the ambulatory care centers and residential treatment facilities.
Documents patient interventions and outcomes in patient’s EHR in accordance with current standards of practice and organizational policies.
Performs efficiently in emergency situations; remains calm, uses good clinical judgment and follows appropriate clinical and organizational protocols for treatment and disposition of patient.
Demonstrates a commitment to maintaining an acceptable level of productivity by:
- Reporting to work on time and in accordance with assigned schedule.
- Facilitating the care of patients in a timely and efficient manner through appropriate triage of scheduled patients as well as walk-ins.
- Demonstrating the ability to recognize priorities in the organization of workflow.
- Assisting other medical providers with heavier patient assignments and emergency situations, when applicable.
Verbalizes and demonstrates knowledge and compliance to DFCC policies and procedures and all current regulatory and statutory requirements.
Participates in the development of standards of care as may be requested by the CMO or DFCC Administration.
Performs within scope of practice, demonstrates awareness of legal issues, and utilizes appropriate judgment and actions to reduce potential adverse consequences for patients, self and the organization.
Participates in meetings, committee activities, peer review process, and the DFCC QI program as assigned.
Assesses personal learning needs and demonstrates a commitment to professional growth by attending continuing education seminars and maintaining current knowledge of health care trends, standards and new technologies.
Provides a safe and secure environment for patients, family, staff and self through the appropriate application of established safety and infection control principles.
Maintains compliance to universal precautions, OSHA standards for blood-borne pathogens and infection control policies.
Meets all organizational requirements for mandatory education at the time of employment and annually, thereafter.
Verbalizes knowledge of and demonstrates sensitivity to the patient’s bill of rights; maintains patient confidentiality at all times and adheres to HIPAA Privacy & Security Rules.
Demonstrates initiative and competency in the utilization of Health Information Technology to perform essential job functions, which include but are not limited to (1) documentation in the EMR, (2) Transmission of EHI, (3) Retrieval of EHI.
Demonstrates ability to assume on-call responsibilities as assigned by the Administrative staff.
Demonstrates respect for the cultural and religious diversities and needs of patients, families and other care providers.
Creates and maintains an atmosphere of courtesy and respect towards patients, families and care providers.
Demonstrates a commitment to building a supportive, team oriented work environment and projects a positive image to the communities we serve.
Requirements:MD or DO degree from an accredited medical school; board certified/eligible in Family Medicine, Internal Medicine or a subspecialty in Internal Medicine, Pediatrics.
NYS license to practice as a physician and a current valid NYS registration.
At least 1 year experience working with indigent populations and community based programs desirable; understands and is able to articulate the concepts of an interdisciplinary team model with a focus on outcome measurements.
Current life support and infection control certificates;
Current DEA certification.
Will meet all the credentialing requirements as stated in the Medical/Dental staff By-Laws and the credentialing processes of Damian Family Care Centers, Inc. (DFCC).
Ability to communicate effectively, clearly, concisely with others both verbally and in writing. Ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial and ethnic backgrounds
Computer literacy with proficiency in Microsoft Word. Experience documenting in an EMR
