We are building a fully integrated care model that includes primary care, behavioral health, and care coordination—ensuring access, dignity, and lifelong support from diagnosis through end-of-life.
Position OverviewWe are seeking a compassionate, motivated Nurse Practitioner (NP) to join our growing clinical team. This provider will play a critical role in delivering high-quality primary care services in a rural setting while helping shape an innovative model of care for vulnerable populations.
This is an opportunity to step into a foundational role within a new clinic with strong leadership support, clinical autonomy, and the ability to make a meaningful impact in the community.
Key Responsibilities- Provide comprehensive primary care services across the lifespan
- Conduct physical exams, assessments, and diagnostic evaluations
- Diagnose and treat acute and chronic conditions
- Prescribe medications and manage treatment plans
- Provide preventative care, screenings, and patient education
- Collaborate with interdisciplinary teams including behavioral health and case management
- Maintain accurate and timely documentation in the EMR
- Ensure compliance with all state, federal, and Rural Health Clinic regulations
- Participate in quality improvement and care coordination initiatives

- Active and unrestricted Kentucky APRN license
- Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology NP
- DEA license (or ability to obtain)
- Experience in primary care, rural health, or underserved populations preferred
- Experience with individuals with I/DD is a strong plus
- Strong clinical judgment and commitment to patient-centered care
