LPN-WOUND CARE NURSE - HIRING BONUS AVAILABLE at Countryside Health Services LLC – Warner, Oklahoma

Countryside Estates in Warner, OK is seeking a Wound Care Nurse for each of our two rotations.

Hiring bonus available.

WOUND CARE NURSE JOB DESCRIPTION

• All new wounds will have a documented initial measurement and assessment performed
• Obtain initial wound care orders from MD and notify resident family members.
• All notifications must be documented in the progress notes.
• Implement interventions for the resident with wounds, such as: Tx orders, dietary supplements, labs, positioning, staff education with documentation of such intervention.
• Notify MDS Coordinator of all new wounds and interventions set in place for the care plan.
• Make weekly rounds with the wound care physician on each resident.
• Weekly wound report and TAR and notify family each week on progress.
• Perform weekly head to toe skin check on all residents for possible skin breakdown and obtain any new orders and measurements as needed with appropriate documentation as well as notifying family.
• Other related duties discussed upon hire.

Active Oklahoma LPN license.
