Your Role:
As a Licensed Practical Nurse (LPN), you’ll provide essential skilled nursing services under the supervision of an RN or physician, ensuring each patient receives personalized care.

Key Responsibilities:

  • Conduct routine nursing tasks: vitals, wound care, medication administration
  • Document all patient care accurately in electronic health records
  • Communicate effectively with patients, families, and the care team
  • Assist with triage calls and follow-ups as needed
  • Coordinate and facilitate referrals and care transitions

Who We're Looking For:

  • Current Oklahoma LPN license
  • Graduate of an accredited LPN program
  • At least one year of experience in home health preferred
  • Excellent communication and clinical judgment
  • Reliable transportation and valid driver’s license

Role Summary:
A Licensed Practical Nurse (LPN) plays a vital role in delivering hands-on patient care under the supervision of a Registered Nurse or physician. This position requires both clinical competence and compassion, as it involves providing essential care to individuals in a home health setting. The ideal candidate is calm under pressure, adaptable, and committed to patient-centered service.

Primary Responsibilities May Include:

  • Monitoring and recording vital signs such as blood pressure, temperature, and weight
  • Performing basic wound care, including cleaning and bandaging
  • Accurately documenting care in electronic health records
  • Answering phones, taking messages, and conducting triage within LPN scope
  • Coordinating patient referrals and follow-up appointments
  • Collaborating effectively with other members of the healthcare team
  • Maintaining confidentiality and adhering to nursing best practices
  • Demonstrating professionalism and compassion in all patient interactions
  • Performing other duties as assigned within the scope of LPN practice

Qualifications:

  • Graduate of an accredited school of practical nursing
  • Active LPN license in the state of Oklahoma
  • One year of home health experience preferred
  • Strong communication and interpersonal skills
  • Proven ability to multitask and work as part of a multidisciplinary team
  • Reliable, organized, and able to maintain composure in challenging situations
