House Calls Provider - Nurse Practitioner - Company Car

Location: Virginia (Field-based; patient homes within service area)

Employment Type: Full-Time

Reports To: Medical Director (Clinical) | Practice Administrator (Operations)

What You’ll Do:

• Provide in-home primary care for chronic disease management, dementia care and preventive health needs
• Develop and manage individualized care plans tailored to patient goals
• Perform cognitive screenings, annual wellness visits, follow-ups, and hospital-to-home transition supervision
• Coordinate care with specialists, home health, hospice, DME providers and community agencies

What You Bring:

• Active Virginia Nurse Practitioner or Physician Assistant
• DEA registration or eligibility
• Medicare PECOS enrolled
• Experience in primary care and geriatrics
