Nurse Practitioner (NP) – Home-Based Primary Care (HBPC)
Location: Virginia (Field-based; patient homes within service area)
Employment Type: W-2 | Full-Time or Part-Time
Reports To: Medical Director (Clinical) | Practice Administrator (Operations)
About the Role
Five Star House Calls is seeking a dedicated Nurse Practitioner to provide longitudinal primary care to medically complex, homebound patients in their homes. This is a field-based role practicing at the top of license under Virginia regulations and HBPC clinical protocols.

What You’ll Do
- Provide in-home primary care for chronic disease management, dementia care, acute issues, and preventive health needs
- Develop and manage individualized care plans tailored to patient goals and clinical complexity
- Perform Annual Wellness Visits (AWVs), follow-ups, Chronic Care Management (CCM), and Transitional Care Management (TCM)
- Coordinate care with specialists, home health, hospice, DME providers, and community agencies
- Document all encounters accurately and promptly in the EHR
- Participate in clinical huddles, case reviews, and quality improvement initiatives
What You Bring
- Active Virginia Nurse Practitioner license
- National certification (AANP or ANCC)
- DEA registration or eligibility
- Medicare and Virginia Medicaid enrollment (preferred)
- Valid driver’s license and reliable transportation
- Experience in primary care, geriatrics, or home-based care (preferred)
