Full-time NP provides in-home primary care to medically complex patients. Be part of a comprehensive care team that includes skilled nursing, therapy, social work, caregiver and dementia care navigator.

Work schedule M-F 9-5. Our office manages your schedule and confirms appointments.

Responsibilities:

• Chronic disease management
• Hospital-to-Home Transition Care Management
• Advanced Care Planning
• Cognitive screenings
• Coordinate care with specialists, home health, hospice and personal care
• Educate patient and family on disease and treatment plan

Qualifications:

• Active Virginia NP license
• Medicare certification or willingness to obtain
• DEA registration or eligibility
• Experience in primary care, geriatrics and home-based care
